目的:探讨在无囊膜的特殊情况下,二期后房型人工晶状体(IOL)植入手术方法。方法:对19例(19眼)患者无后囊膜支持的情况下进行二期后房人工晶体植入。采用巩膜层间无缝线人工晶体固定术,三片式人工晶体折叠后由透明角膜切口注入,晶体襻由角膜缘后2 mm的巩膜瓣下穿刺口穿出,置入预先做好的巩膜层间穿刺隧道内。术后随访半年以上,观察眼压、视力、人工晶体稳定性及并发症。结果:术后19例患者视力均明显提高,视力>0.6者13例,0.2~0.6者6例,眼压稳定,人工晶体居中良好,无偏斜及移位,随访期间无晶体脱位,眼内炎、低眼压、角膜失代偿等严重并发症。结论:在晶状体囊膜缺失眼,巩膜层间无缝线固定人工晶体术,是一种可行有效的手术。
目的:探討在無囊膜的特殊情況下,二期後房型人工晶狀體(IOL)植入手術方法。方法:對19例(19眼)患者無後囊膜支持的情況下進行二期後房人工晶體植入。採用鞏膜層間無縫線人工晶體固定術,三片式人工晶體摺疊後由透明角膜切口註入,晶體襻由角膜緣後2 mm的鞏膜瓣下穿刺口穿齣,置入預先做好的鞏膜層間穿刺隧道內。術後隨訪半年以上,觀察眼壓、視力、人工晶體穩定性及併髮癥。結果:術後19例患者視力均明顯提高,視力>0.6者13例,0.2~0.6者6例,眼壓穩定,人工晶體居中良好,無偏斜及移位,隨訪期間無晶體脫位,眼內炎、低眼壓、角膜失代償等嚴重併髮癥。結論:在晶狀體囊膜缺失眼,鞏膜層間無縫線固定人工晶體術,是一種可行有效的手術。
목적:탐토재무낭막적특수정황하,이기후방형인공정상체(IOL)식입수술방법。방법:대19례(19안)환자무후낭막지지적정황하진행이기후방인공정체식입。채용공막층간무봉선인공정체고정술,삼편식인공정체절첩후유투명각막절구주입,정체반유각막연후2 mm적공막판하천자구천출,치입예선주호적공막층간천자수도내。술후수방반년이상,관찰안압、시력、인공정체은정성급병발증。결과:술후19례환자시력균명현제고,시력>0.6자13례,0.2~0.6자6례,안압은정,인공정체거중량호,무편사급이위,수방기간무정체탈위,안내염、저안압、각막실대상등엄중병발증。결론:재정상체낭막결실안,공막층간무봉선고정인공정체술,시일충가행유효적수술。
Objective:To investigate the surgery of secondary posterior chamber intraocular lens implantation in special cases of capsule coloboma.Method:Nineteen eyes of 19 patients with capsule coloboma underwent secondary posterior chamber intraocular lens implantation which were fixed in the intra sclera and sutureless,the folded three-pieces of intraocular lens were injected into the posterior chamber through the incision on the clear cornea,the loops passed out though the scleral flap 2 mm after the limbus and went into the pre-made scleral tunnel incision.Postoperative follow-up included intraocular pressure,best-corrected visual acuity,IOL stability,and other postoperative complications for over 6 months.Result:Nineteen eyes of 19 patients were involved in the study,visual acuity after surgery was better than 0.6 in 13 eyes,between 0.2 and 0.6 in 6 eyes,all patients experienced obvious vision improvement with normal intraocular pressure,the IOLs were centered without tilt or decentration,no severe complications such as endophthalmitis,low intraocular pressure,coneal decompensatory were observed during the follow-up.Conclusion:Sutureless intrascleral posterior chamber intraocular lens implantation is a reliable and effective therapy for eyes without posterior capsular support.
