目的研究玻璃体切割术(PPV)对增殖性糖尿病视网膜病变(PDR)的临床疗效。方法回顾性分析行玻璃体切割术的83例(95眼)PDR患者的临床资料。患者均行玻璃体切割术加眼内光凝术,并根据具体情况加行巩膜外环扎、巩膜外硅胶垫压、C3F8填充或硅油填充。结果术后视力改善56只眼,其中Ⅳ期11只眼(84.62%),Ⅴ期24只眼(64.86%),Ⅵ期21只眼(46.67%)。不同分期的糖尿病视网膜病变,其术后视力改善的差异有统计学意义(x2=6.878,P<0.05);术后主要并发症有继发性高眼压、玻璃体腔再出血、并发性白内障。结论玻璃体切割术是治疗增殖性糖尿病视网膜病变的有效手段;尽早手术治疗对增殖性糖尿病视网膜病变愈后有重要意义。

목적연구파리체절할술(PPV)대증식성당뇨병시망막병변(PDR)적림상료효。방법회고성분석행파리체절할술적83례(95안)PDR환자적림상자료。환자균행파리체절할술가안내광응술,병근거구체정황가행공막외배찰、공막외규효점압、C3F8전충혹규유전충。결과술후시력개선56지안,기중Ⅳ기11지안(84.62%),Ⅴ기24지안(64.86%),Ⅵ기21지안(46.67%)。불동분기적당뇨병시망막병변,기술후시력개선적차이유통계학의의(x2=6.878,P<0.05);술후주요병발증유계발성고안압、파리체강재출혈、병발성백내장。결론파리체절할술시치료증식성당뇨병시망막병변적유효수단;진조수술치료대증식성당뇨병시망막병변유후유중요의의。
Objective: To study the effect of vitrectomy ( PPV ) on proliferative diabetic retinopathy ( PDR ) clinical curative effect. Methods: a retrospective analysis of 83 cases of vitrectomy (95 eyes ) of the clinical data of patients with PDR. Patients underwent vitrectomy combined with intraocular photocoagulation, and according to the specific case underwent scleral encircling, scleral silica gel pad, C3F8 filling or silicone oil tamponade. Results: postoperative improvement in 56 eyes, including 11 eyes ( 84.62% ) at stage Ⅳ,Ⅴ stage in 24 eyes ( 64.86% ), 21 eyes ( 46.67% ) stage VI. Different stages of diabetic retinopathy, the difference was statistically significant . The postoperative visual acuity improved ( x2=6.878, P<0.05); the main postoperative complications were secondary high IOP, vitreous hemorrhage, complicated cataract. Conclusion: vitrectomy is an effective method in the treatment of proliferative diabetic retinopathy; early operation treatment is important for proliferative diabetic retinopathy after healing.
