目的:观察地佐辛用于硬膜外麻醉剖宫产术中的镇痛效果和安全性。方法:100例拟在连续硬膜外麻醉行剖宫产手术的产妇,ASAI~Ⅱ级、单胎、足月妊娠,随机分为两组,每组各50例。对照组常规硬膜外麻醉下行剖宫产;实验组在完成硬膜外麻醉后静推5mg地佐辛。分别记录两组产妇在切皮时(T1),牵拉取胎时(T2),探查腹腔时(T3),关腹缝皮时(T4)的视觉模拟评分(VAS);脉搏氧饱和度(SPO2);观察两组新生儿出生后l、3、5min的Apgar评分。结果:术中两组产妇的SPO2均在正常值范围内,没有统计学差异。与对照组比较,T1、T4时间点VAS评分≥4人数无统计学差异(P>0.05);T2、T3点实验组VAS评分≥4人数显著降低(P<0.05)(表1)。新生儿出生后1、3、5min的Apgar评分均正常,两组之间比较无统计学差异(P>0.05)。结论:小剂量地佐辛用于硬膜外麻醉剖宫产术中的镇痛效果较好,对产妇及新生儿无明显不良影响。

목적:관찰지좌신용우경막외마취부궁산술중적진통효과화안전성。방법:100례의재련속경막외마취행부궁산수술적산부,ASAI~Ⅱ급、단태、족월임신,수궤분위량조,매조각50례。대조조상규경막외마취하행부궁산;실험조재완성경막외마취후정추5mg지좌신。분별기록량조산부재절피시(T1),견랍취태시(T2),탐사복강시(T3),관복봉피시(T4)적시각모의평분(VAS);맥박양포화도(SPO2);관찰량조신생인출생후l、3、5min적Apgar평분。결과:술중량조산부적SPO2균재정상치범위내,몰유통계학차이。여대조조비교,T1、T4시간점VAS평분≥4인수무통계학차이(P>0.05);T2、T3점실험조VAS평분≥4인수현저강저(P<0.05)(표1)。신생인출생후1、3、5min적Apgar평분균정상,량조지간비교무통계학차이(P>0.05)。결론:소제량지좌신용우경막외마취부궁산술중적진통효과교호,대산부급신생인무명현불량영향。
Objective:To observe dezocine epidural anesthesia for cesarean section in the analgesic efficacy and safety. Methods:100 cases contemplated in continuous epidural anesthesia for cesarean delivery of maternal, ASAI~Ⅱstage, single tires, ful-term pregnancy were randomly divided into two groups, each 50 cases. Conventional control group cesarean epidural anesthesia;experimental group after the completion of bolus epidural anesthesia 5mg dezocine. Mothers in both groups were recorded at skin incision (T1), pul ing the fetus when (T2), abdominal exploration time (T3), the abdomen skin closure (T4) visual analogue scale (VAS);pulse oxygen saturation (SPO2);were observed after birth, l, 3,5 min Apgar score. Results:Intraoperative two maternal SPO2 were within normal range, there is no statistical y significant difference. Compared with control group, T1, T4 VAS score≥4 point in time the number was no significant difference (P>0.05);T2, T3 point VAS score≥4 experimental group significantly reduced the number (P<0.05) (Table 1). 1,3,5 min after birth, Apgar scores were normal, no statistical y significant difference between the two groups (P>0.05). Conclusion:Low dose dezocine epidural anesthesia for cesarean section analgesia bet er, on maternal and neonatal no significant adverse effects.
