目的：探讨改良输尿管镜联合膀胱镜治疗输尿管中下段大结石的有效性及安全性。方法：采用输尿管镜钬激光碎石术治疗输尿管中下段大结石35例，术中尿道同时留置12F导尿管，结石碎至3~4mm，全部钳夹于膀胱，使用22F膀胱镜鞘，Toomy空针吸出全部碎石。结果：所有患者手术用时25~50min，平均35min，未见残余结石及输尿管穿孔、狭窄、粘膜撕脱等并发症。结论：留置12F导尿管不影响输尿管镜操作，联合膀胱镜治疗输尿管中下段大结石，能明显缩短手术时间，不增加手术并发症，值得进一步推广。
目的：探討改良輸尿管鏡聯閤膀胱鏡治療輸尿管中下段大結石的有效性及安全性。方法：採用輸尿管鏡鈥激光碎石術治療輸尿管中下段大結石35例，術中尿道同時留置12F導尿管，結石碎至3~4mm，全部鉗夾于膀胱，使用22F膀胱鏡鞘，Toomy空針吸齣全部碎石。結果：所有患者手術用時25~50min，平均35min，未見殘餘結石及輸尿管穿孔、狹窄、粘膜撕脫等併髮癥。結論：留置12F導尿管不影響輸尿管鏡操作，聯閤膀胱鏡治療輸尿管中下段大結石，能明顯縮短手術時間，不增加手術併髮癥，值得進一步推廣。
목적：탐토개량수뇨관경연합방광경치료수뇨관중하단대결석적유효성급안전성。방법：채용수뇨관경화격광쇄석술치료수뇨관중하단대결석35례，술중뇨도동시류치12F도뇨관，결석쇄지3~4mm，전부겸협우방광，사용22F방광경초，Toomy공침흡출전부쇄석。결과：소유환자수술용시25~50min，평균35min，미견잔여결석급수뇨관천공、협착、점막시탈등병발증。결론：류치12F도뇨관불영향수뇨관경조작，연합방광경치료수뇨관중하단대결석，능명현축단수술시간，불증가수술병발증，치득진일보추엄。
Objective:To investigate the effectiveness and safety of modified combined ureteroscope and bladder endoscopy in the treatment of ureteral stones.Methods:35 cases treated with ureteroscopic holmium laser lithotripsy in treatment of ureteral stones, urethral and intraoperative 12F catheter, stone crushing to 3~4mm, al the clamp in the bladder, the use of 22F cystoscope sheath, Toomy needle aspiration al broken stone. Results:Al of the patients the operation time was 25~ 50min, average 35min, no residual stone and ureteral perforation, narrow, mucosal avulsion and other complications. Conclusion:12F indwel ing catheter does not affect the ureteroscope operation, combined with cystoscope for middle and lower ureteral stones, can shorten the operation time, does not increase the operation complications, worthy of further promotion.
