目的:探讨瑞芬太尼插管期间听觉诱发电位和血流动力学变化,评价瑞芬太尼抑制气管插管时心血管的副反应,为临床寻求一种安全有效的方法。方法:将60例ASAI-II级患者分为两组,麻酸诱导前4min,观察组(I组)静注瑞芬太尼1μg/kg后,以BaxterAS40A型向量泵持续静脉注瑞芬太尼0.1μg/(kg.min),诱导时,瑞芬太尼改为0.05μg/(kg.min)。对照组(II组)以持续静注艾司洛尔300μg/(kg.min),诱导时改为200μg/(kg.min)。后静注麻醉诱导剂,待肌肉完全松驰时行气管内插管,至持管后10min停药。比较两组治疗期间AAL、ECG、HR、SBP、DBP、RPP,经统计学处理。结果:两组患者各时间点的AAI值无显著性差异(P>0.05),观察组ISBP、HR、RPP在气管插管后的全过程变小。结论:瑞芬太尼静注能够有效地抑制气管插管时的心血管副反应。

목적:탐토서분태니삽관기간은각유발전위화혈류동역학변화,평개서분태니억제기관삽관시심혈관적부반응,위림상심구일충안전유효적방법。방법:장60례ASAI-II급환자분위량조,마산유도전4min,관찰조(I조)정주서분태니1μg/kg후,이BaxterAS40A형향량빙지속정맥주서분태니0.1μg/(kg.min),유도시,서분태니개위0.05μg/(kg.min)。대조조(II조)이지속정주애사락이300μg/(kg.min),유도시개위200μg/(kg.min)。후정주마취유도제,대기육완전송치시행기관내삽관,지지관후10min정약。비교량조치료기간AAL、ECG、HR、SBP、DBP、RPP,경통계학처리。결과:량조환자각시간점적AAI치무현저성차이(P>0.05),관찰조ISBP、HR、RPP재기관삽관후적전과정변소。결론:서분태니정주능구유효지억제기관삽관시적심혈관부반응。
Objective:To explore the changes of AAI and hemodynamic during anesthesia induction by remifentanil to evaluate the restrain effect of remifentanil to adverse reactions during anesthesia induction. Methods:60 cases of ASAI-II patients were divided into 2 groups,observation group 1(remifentanil,n=30) and control group II (emolol,n=30) .Remifentanil in group I was injected intravenously with 1μg/kg and then continuous infuction with 0.1μg/(kg.min) at 4 min before the anaesthesia induces and was remifentanil was injucted with 0.05μg/(kg.min)at the time of anesthesia induces.Continuous intravenous injection in control group II (emolol,n=30)with 300μg/(kg.min),rised to 200μg/(kg.min) at induction of anesthesia.To compare AAI.ECG.HR.SBP and RPP in two groups.Results:The values of AII in two groups did not have significant difference (P>0.05),BP.HR and RPP changed lit le in group I.Conclusion:Remifentanil can more effectively inhibit cardiovascular side effects during tracheal intubation.
