目的 比较分析胃癌与正常人血清蛋白质谱的变化,筛选特异性蛋白标志物,建立胃癌诊断分类树模型.方法 收集血清样本231例(其中胃癌116例,正常115例),随机分为建模组和验证组.应用弱阳离子磁珠联合基质辅助激光解吸离子化飞行时间质谱,建立胃癌与正常人血清蛋白质谱,对建模组43例胃癌和42例正常人血清差异蛋白质谱进行定量分析,应用GeneticAlgorithm算法建立胃癌诊断模型,应用所获取的诊断模型对验证组样本(73例胃癌、73例正常人)进行分类诊断,以评价诊断模型的诊断价值.结果 通过比较分析胃癌与正常人血清蛋白质谱,发现共有48个差异蛋白峰(P<0.05),其中在胃癌中表达上调25个,表达下调23个,利用其中8个差异峰(质荷比分别为3 316.28、1 213.99、2 934.23、1 214.86、760.02、1 779.70、3 884.85及1 945.67)建立诊断模型,准确率为94.11%,经独立样本双盲验证,其灵敏度为93.15%,特异度为94.52%.结论 基于磁珠分离和质谱技术能直接检测出胃癌患者血清差异表达蛋白,建立的诊断模型具有较高的敏感性和特异性,对提高胃癌的诊断具有一定的临床意义.

Objective To detect the specific protein biomarkers of gastric cancer by exploring the difference serum proteomic spectra between patients with gastric cancer and healthy individuals.Methods Serum samples of 116 cases of gastric cancer patients and 115 cases of healthy individuals were collected,and randomized divided into model construction group and validation group.Magnetic bead kit and mass spectrometry were used to detect serum samples from patients with gastric cancer and healthy individuals.The model was then built by ClintProt tools 2.2 Software and was further evaluated in blind test for reliability.Results Fourty-eight differentially expressed proteins in serum were detected by analysis of protemic spectra of gastric cancer patients and healthy individuals,including 25 up-regulated proteins and 23 downregulated ones.Eight proteins (m/z 3 316.28,1 213.99,2 934.23,1 214.86,760.02,1 779.70,3 884.85 and 1 945.67) were obtained for making up ClinProt Model which could classify the gastric cancer patients and healthy individuals with an accuracy of 94.11%.By a double blind validation,the ClintProt Model yielded a sensitivity of 93.15% and a specificity of 94.52%,respectively.Conclusion Combination of magnetic bead kit and mass spectrometry has clinical significance for improving the accuracy of gastric cancer diagnosis.
