目的::观察自拟肠神方预防奥沙利铂慢性周围神经毒性的临床疗效。方法:选取病理明确,接受姑息性化疗(mFOLFOX6)的晚期结肠癌患者64例,随机分为治疗组42例,化疗联合肠神方内服外洗;对照组22例,化疗联合钙镁合剂治疗。疗程结束后评估周围神经毒性和疗效,并检测周围感觉神经传导速度和动作电位波幅。结果:10个周期化疗后,两组各级周围神经毒性发生率差异有统计学意义(P<0.05),且治疗组感觉神经传导速度和动作电位波幅的下降程度明显小于对照组(P<0.01)。两组肿瘤治疗效果无明显差异(P>0.05)。结论:肠神方内服外洗可以有效预防奥沙利铂引起的慢性周围神经毒性,对周围感觉神经功能有一定的保护作用,并且不影响化疗效果。

목적::관찰자의장신방예방오사리박만성주위신경독성적림상료효。방법:선취병리명학,접수고식성화료(mFOLFOX6)적만기결장암환자64례,수궤분위치료조42례,화료연합장신방내복외세;대조조22례,화료연합개미합제치료。료정결속후평고주위신경독성화료효,병검측주위감각신경전도속도화동작전위파폭。결과:10개주기화료후,량조각급주위신경독성발생솔차이유통계학의의(P<0.05),차치료조감각신경전도속도화동작전위파폭적하강정도명현소우대조조(P<0.01)。량조종류치료효과무명현차이(P>0.05)。결론:장신방내복외세가이유효예방오사리박인기적만성주위신경독성,대주위감각신경공능유일정적보호작용,병차불영향화료효과。
Objective:To observe the therapeutic effect of self-designed Changshen prescription in the treatment of peripheral neurotoxicity caused by oxaliplation. Methods:Sixty-four patients with colon cancer at the end stage confirmed by pathology were randomly divided into control(22 patients) and Changshen(42 patients) groups. All pa-tients received palliative chemotherapy with mFOLFOX6R. Patients in the Changshen group was administered Changshen prescription orally and externally in addition and patients in the control group were given plus Ca/Mg infusion. Nerve toxicity and therapeutic effect was evaluated and peripheral sensory nerve conduction velocity(SCV) and action potential(SNAP) were tested in both groups after the full treatment. Results:After 10 cycles of chemother-apy,the patient in the Changshen group had an incidence of 85.3% of neurotoxicity,significantly lower than that of the control group(94.4%,P<0.05),and they also had a smaller reduction in SCV and SNAP(P<0.01). The thera-peutic effect did not differ between the two groups(P>0.05). Conclusion:Applying Changshen prescription both o-rally and externally can effectively prevent patients from chronic peripheral neurotoxicity caused by oxaliplation and have protective effect on sensory nerve function,with the same anti-tumor effect as Ca/Mg infusion has.
