目的 评价丝裂霉素C (MMC)对翼状胬肉自体角膜缘干细胞移植术疗效的影响.方法 将241例(317只眼)患者随机分为两组,MMC组183只眼,其中原发性胬肉123只眼,复发性胬肉60只眼;对照组134只眼,其中原发性胬肉94只眼,复发性胬肉40只眼.两组均常规行翼状胬肉切除联合自体角膜缘干细胞移植,MMC组术中使用0.02%MMC棉片4 min.对照组不使用任何药物.定期随访12~24个月,观察胬肉复发情况及术后并发症.结果 原发性胬肉组及复发性胬肉组,胬肉复发率MMC组均低于对照组,眼部刺激症状、角膜上皮修复时间及移植片愈合时间两组差异均无统计学意义(P>0.05).结论 翼状胬肉切除并自体角膜缘干细胞移植术中应用0.02% MMC可起到协同作用,能进一步降低术后复发率,对复发性胬肉更具有意义.无毒副反应产生.

Objective To observe the curative effect of using mitomycin C in pterygium excision with autologous limbus stem cell transplantation.Methods A total of 241 cases (317eyes)were randomly divided into two groups,0.02% MMC cotton patch was putting on the sclera for 4 minutes during the operation in 183 eyes of MMC group (the primary pterygium 123 eyes,the recurrent pterygium 60 eyes); The control group 134 eyes (the primary pterygium 94 eyes,the recurrent pterygium 40 eyes) did not use any medication.All patients were followed up for12-24 months to observe the recrudescence and complications after surgery.Results The recurrence rate in MMC group was fewer than that of control group,the statistical difference was more significant in the re current pterygium (P <0.01) than that in the primary pterygium (P <0.05).There was no statistical difference of complications between the 2 groups (P >0.05).Conclusions The application of 0.02%MMC during the pterygium excision with autologous limbus stem cell transplantation can more reduce the recurrence rate of pterygium,especially for the recurrent pterygium.There is no side effect.
