目的:从服务利用方的角度,对瑞金-卢湾医联体内部就诊流程的合理性进行初步分析和评价,为卫生行政部门提供决策依据。方法:采用方便抽样的方法对门诊患者进行问卷调查和深入访谈,通过情景模拟了解患者对医联体就诊流程中“社区首诊”、“双向转诊”、“绿色通道”和“预约机制”4个关键环节的认知和看法。结果:总共调查306人。86.9%看病首选社区,主要是因为“交通方便”、“一些小病不用到大医院”、“社区医生服务态度好”、“费用便宜”;41.2%认为“向上转诊须由专家审核”的规定不合理,担心增加专家审核会使转诊更加麻烦,病情在此期间可能发生转变;90.1%支持社区康复,认为下转到社区进行康复可以节省费用;96.1%赞同开通绿色通道,但利用情况较差;75.3%接受预约服务。结论:瑞金-卢湾医联体内部就诊流程总体上较为合理,但需(1)提升服务质量,引导居民社区首诊;(2)优化转诊流程,促进服务系统高效运转;(3)采用多种服务手段,满足居民多样化需求。

Objective:To analyze and evaluate the reasonability of treatment process of Ruijin-Luwan Medical Association from the point of service utilization,and to provide decision-making support for health administration departments. Methods:This research employed a convenience sampling approach to investigate outpatients with questionnaire and in-depth interviews. Respondents’ cognition and attitude toward“community consultation firstly”,“two-way referral”,“green passage”and“appointment mechanism”were stimulated and analyzed. Results:A total of 306 patients were investigated,86.9%preferred choosing community health service firstly because of the“convenient transportation”,“a common disease with no need to go to larger hospitals”,“the community physician who can provide good service”,as well as“a lower expenditure”. A total of 41.2% of them didn’t think the rule reasonable,which appointed an expert to review the referral application,worrying about cumbersome procedures and the deterioration of disease during the process. Totally 90.1%supported community-based rehabilitation for reducing expenses and 96.1%were in favor of building green passage,but the use of it was inadequate,and 75.3%can accept appointment services. Conclusion:Overall,the internal treatment process of Ruijin-Luwan Medical Association is reasonable to some extent. However,more attention should be paid as follows:(1)improving the quality of service and prompting a change of people’s health concept and behavior;(2)optimizing the referral process and promoting the efficient operation of delivery system;(3)providing different kinds of services to satisfy various needs of residents.
