目的:探讨磁共振定量动态增强鉴别乳腺良恶性病变的价值。方法收集39例行1.5 T磁共振定量动态增强扫描的乳腺疾病患者为对象,测得病变的定量参数容量转移常数(Ktrans )、速率常数(Kep )、血管外细胞外间隙容积比(Ve )值。据术后或穿刺病理结果分为良恶性病变组,并取患者自身对侧正常的乳腺组织为对照组,对3组定量参数进行统计学分析。结果恶性病变组、良性病变与对照组Ktrans 、Kep平均值依次减低,且两两比较具有统计学意义;恶性病变组、良性病变与对照组 Ve值依次升高,进一步两两比较,仅恶性病变组与良性病变组、恶性病变组与对照组间Ve值差别有统计学意义。恶性病变组中浸润性导管癌和导管内癌的Ktrans 、Kep 、Ve值的差别无统计学意义。良性病变组中伴有不典型增生和非不典型增生的Ktrans 、Kep值的差别有统计学意义,Ve值的差别无统计学意义。结论磁共振定量动态增强参数Ktrans 、Kep 、Ve值对乳腺良恶性病变鉴别诊断具有意义。

목적:탐토자공진정량동태증강감별유선량악성병변적개치。방법수집39례행1.5 T자공진정량동태증강소묘적유선질병환자위대상,측득병변적정량삼수용량전이상수(Ktrans )、속솔상수(Kep )、혈관외세포외간극용적비(Ve )치。거술후혹천자병리결과분위량악성병변조,병취환자자신대측정상적유선조직위대조조,대3조정량삼수진행통계학분석。결과악성병변조、량성병변여대조조Ktrans 、Kep평균치의차감저,차량량비교구유통계학의의;악성병변조、량성병변여대조조 Ve치의차승고,진일보량량비교,부악성병변조여량성병변조、악성병변조여대조조간Ve치차별유통계학의의。악성병변조중침윤성도관암화도관내암적Ktrans 、Kep 、Ve치적차별무통계학의의。량성병변조중반유불전형증생화비불전형증생적Ktrans 、Kep치적차별유통계학의의,Ve치적차별무통계학의의。결론자공진정량동태증강삼수Ktrans 、Kep 、Ve치대유선량악성병변감별진단구유의의。
Objective To investigate the value of quantitative dynamic contrast-enhanced MRI in characterizing breast lesions as malignant or benign . Methods Thirty-nine Patients underwent quantita-tive dynamic contrast-enhanced MRI at 1.5T and had pathology-proven diagnosis . Lesions were identi-fied using pharmacokinetic parameters :Ktrans (volume transfer constant ) ,Kep (reverse volume transfer con-stant) ,and Ve (the extravascular extracellular space volume per unit volume of tissue ) . Mean Ktrans ,Kep and Ve of malignant ,benign and normal glandular tissues were analyzed . Results Mean Ktrans and Kep was higher in malignant tumors than that in benign lesions . Mean Ktrans and Kep tissues was lower in normal glandular than that in benign lesions . There were significant differences between malignant and benign lesions ,malignant lesions and normal glandular tissues ,benign lesions and normal glandular tissues in Ktrans and Kep . Mean Ve was lower in malignant tumors than that in benign lesions . Mean Ve was high-er in normal glandular tissues than that in benign lesions . There were significant differences between ma-lignant and benign lesions ,malignant lesions and normal glandular tissues in Ve. There were not signifi-cant differences between invasive ductal carcinoma and ductal carcinoma in situ in K trans ,Kep and Ve . There were statistic differences between atypical hyperplasia and non-atypical hyperplasia in Ktrans and Kep. Conclusion Quantitative dynamic contrast-enhanced MRI has potential role in estimating the benign and malignant breast lesions.
