目的探讨肝后下腔静脉前间隙的解剖结构,为临床绕肝提拉切除术建立肝后隧道提供解剖学基础。方法选取2010~2012年期间固定后的成人尸肝38例,用乳胶灌注下腔静脉。打开下腔静脉,测量下腔静脉前间隙内的直径超过1mm的肝短静脉;解剖并观察下腔静脉前间隙的结构。结果肝后下腔静脉与肝之间包有纤维组织膜即下腔静脉韧带;下腔静脉前间隙的左、右边界分别为尾状叶静脉与肝中静脉右缘的连线,肝右后下静脉(IRHV)与肝右静脉左缘的连线;38例标本中23例出现IRHV、肝右后中静脉(MRHV)或二者同时存在,在所有的标本中都存在 Spiegel叶静脉;在下腔静脉前间隙内,有少量细小的肝短静脉,均位于肝后下腔静脉的中、下1/3处,88.6%的位于下腔静脉的左前壁汇入,11.4%的位于下腔静脉前壁或稍偏右处汇入。结论肝后下腔静脉前间隙可以建立肝后隧道,在肝脏外科绕肝提拉法肝切除术中具有重要的临床意义。

목적탐토간후하강정맥전간극적해부결구,위림상요간제랍절제술건립간후수도제공해부학기출。방법선취2010~2012년기간고정후적성인시간38례,용유효관주하강정맥。타개하강정맥,측량하강정맥전간극내적직경초과1mm적간단정맥;해부병관찰하강정맥전간극적결구。결과간후하강정맥여간지간포유섬유조직막즉하강정맥인대;하강정맥전간극적좌、우변계분별위미상협정맥여간중정맥우연적련선,간우후하정맥(IRHV)여간우정맥좌연적련선;38례표본중23례출현IRHV、간우후중정맥(MRHV)혹이자동시존재,재소유적표본중도존재 Spiegel협정맥;재하강정맥전간극내,유소량세소적간단정맥,균위우간후하강정맥적중、하1/3처,88.6%적위우하강정맥적좌전벽회입,11.4%적위우하강정맥전벽혹초편우처회입。결론간후하강정맥전간극가이건립간후수도,재간장외과요간제랍법간절제술중구유중요적림상의의。
Objective To explore the retrohepatic IVC space anatomical structure to provide the morphological data for set ing up a retrohepatic tube through the liver hanging maneuver. Methods 38 adult livers were taken from cadavers during the 2010-2012 year, af use emulsion into inferior vel a cava. Open the inferior vena cava, measuring the short hepatic vein with diameter over 1mm in IVC space; Dissect and observe the space structure before IVC. Results Fibrous membrane, that is, inferior vel a cava Ligament was found between retrohepatic IVC and liver;the two sides of space before inferior vena cava was separately connections bewteen Spiegel veins and middle hepatic veins, IRHV and right hepatic veins;Among 38 cases, IRHV or MRHV, or both of them occurred in 23 patients, Spiegel veins existed in al specimens;In the space before IVC, there are a few short hepatic veins, located in the middle or at 1/3 of retrohepatic IVC, 88.6% into left anterior wal , 11.4% into the anterior wal or slightly right. Conclusions The results show that retrohepatic tunnel can be set up in the space before retrohepatic IVC, which has important clinical significance in liver surgery.
