目的探讨检测早产高危孕妇宫颈分泌物中高磷酸化胰岛素样生长因子结合蛋白-1(ph IGFBP -1)、会阴超声检查宫颈长度预测早产的价值。方法孕28~36w用分娩预测试纸检测宫颈分泌物中 ph IGFBP -1(Actim Partus test),同时会阴超声检测宫颈长度( cervical length, CL),观察记录分娩结局。结果IGFBP-1、宫颈指数阳性组中早产发生率明显高于阴性组(P<0.01),该两指标联合应用对早产的阳性预测值明显提高(P<0.01)。两指标阳性组48h内分娩率为50%,1w内分娩率为70%,距离分娩时间明显短于单一指标阳性组及两指标阴性组(P<0.01)。而两指标阴性组2w内分娩的阴性预测值为100%。血清CRP在早产组与足月产组间无统计学差异(P>0.05)。结论宫颈长度联合ph IGFBP-1检测对预测早产有较高的价值。

Objective To investigate the detection of high risk premature cervical secretions in highly phosphorylated insulin-like growth factor binding protein - 1 ( pH IGFBP 1 ), perineal ultrasound examination of cervical length for predicting preterm delivery value. Methods Between 28 -36 weeks for delivery as predictors of test paper for detecting the cervical secretion in pH IGFBP 1 ( Actim Partus test ), at the same time transperineal ultrasound detection of cervical length ( cervical length, CL ), observe and record the outcome of childbirth. Results The incidence of preterm birth was significantly higher than that of negative group, cervical index in IGFBP-1 positive group ( P<0.01 ), positive predictive joint application of the two indexes of preterm birth was significantly increased ( P< 0.01 ). The two index was positive in 48 hours delivery rate was 50%, within 1 weeks of delivery rate was 70%, distance delivery time was shorter than the single index positive group and negative group two index ( P<0.01). Negative and two indexes of negative group within 2 weeks of delivery of the predictive value of 100%. Serum CRP showed no significant dif erence in the preterm group and term birth group ( P>0.05 ). Conclusion Cervical length and pH IGFBP-1 test in predicting preterm birth have a higher value.
