目的:探讨自发荧光支气管镜( AFB)在中央型支气管肺癌诊断中的价值。方法对93例临床怀疑支气管肺癌患者采用AFB联合白光支气管镜( WLB)检查的诊断结果进行回顾性分析,统计AFB联合WLB和WLB的镜下异常表现及病理结果,比较两种方法诊断早期中央型肺癌的灵敏度、特异度。结果93例患者中,AFB联合WLB检查表现异常者79例,WLB检查表现异常者59例;74例患者病理诊断为阳性,其中重度非典型增生3例,原位癌1例,浸润性癌70例。 AFB联合WLB检查发现异常病理证实阳性者67例,WLB检查发现异常病理证实阳性者50例;两者诊断中央型肺癌的灵敏度分别为90?5%、67?6%,差异有统计学意义(P<0?05)。结论对于中央型支气管肺癌的定位诊断,AFB 检查的灵敏度比 WLB更佳。

Objective To investigate the value of using autofluorescence bronchoscopy( AFB) for the detection and localiza-tion of central type early lung cancer. Methods 93 patients,suspected of lung cancer were examinated by AFB and white light bron-choscopy( WLB) . All of the suspicious abnormal sites were recorded and biopsied for pathological examination. Two methods were com-pared by sensitivity and specificity. Results Included 93 biopsy specimens,and 74 were classified as positive by pathology,including 70 invasive cancer,1 carcinoma in situ and 3 severe dysplasia. AFB combined with WLB found positive resuts comfirmed by pathology in 67 cases, and WLB alone found positive in 50 cases. The relative sensitivity to detect central type early lung cancer of WLB com-bined with AFB versus WLB alone was 90?5% and 67?6%, respectively,with statistical difference significantly( P<0?05) . Conclusion The sensitivity of AFB is better than WLB in detection and localization of central type early lung cancer.
