目的:探讨MRI动态增强血管成像中单侧乳腺血供增加对同侧乳腺癌的诊断价值。资料与方法对169例乳腺疾病患者行乳腺动态增强磁共振检查,在最大密度投影图像上进行血管计数,当血管长度≥3c m且最大直径≥2m m时,列入筛选合格范围,得到两侧的差异血管数目,即血管得分。两侧乳腺合格血管差值≥2个时认为单侧乳腺血供增加。分析乳腺单侧血供增加诊断同侧乳腺癌的灵敏度、特异度,以及与病灶大小的关联。结果以乳腺单侧血供增加作为同侧乳腺癌的诊断标准的灵敏度为65%,特异度为84%,阳性似然比(+LR)为4.06,阴性似然比(-LR)为0.42。T3肿瘤的平均血管评分显著高于T1和T2肿瘤。结论 MRI动态增强血管成像可以很好的显示双侧乳腺整体血供,单侧乳腺血供增加与同侧乳腺癌相关。

Objective This study was done to estimate the diagnostic performance of one-sided vascular prominence foripsilateral breast cancer. Materials and Methods A total of 169 patients underwent bilateral contrastenhanced magnetic resonance imaging. Vessels ≥2mm in diameter and ≥3cm in length were counted on maximum intensity projections. a difference ≥2 in number between the two breasts was considered one-sided vascular prominence. the association of one-sided vascular prominence with ipsilateral breast cancerand the diameter of invasive lesions were analysedusing Maximum intensity projections. ResultsWhen a difference of at least two vessels was scored as vascular asymmetry, the sensitivity, specificity, positive likelihood ratio, and negative of unilateral y increased vascularity associated with ipsilateral malignancy were 69%, 92%, 8.72, and 0.34, respectively. ConclusionMRI vascular mapping is an effective method for determining breast tissue vascularization. Ipsilateral increased vascularity was commonly associated with malignant breast lesions.
