Pulmonary Ventilation
[目的]观察单肺通气(one-lung ventilation ,OLV )期间雾化吸入长托宁对肺内分流率(Qs/Qt)、氧合指数(oxygenation index ,OI)和血乳酸(blood lactic acid ,BLA)水平的影响。[方法]选择28例OLV行开胸手术的患者,随机分长托宁组(P组,于O L V期间雾化吸入长托宁)和对照组(C组)。分别于O L V前15 min(T0)、OLV 15 min(T1)、OLV 45 min(T2)抽动脉血与混合静脉血行血气分析及BLA测定,计算Qs/Qt与OI 。[结果]OLV后两组OI均下降,但P组较C组下降缓慢,且P组T2较T1有所回升;OLV后两组Qs/Qt和BLA均增加,但P组比C组增加较慢,且P组T2较T1有所下降。[结论]OLV期间雾化吸入长托宁能改善氧合,降低肺内分流。

[목적]관찰단폐통기(one-lung ventilation ,OLV )기간무화흡입장탁저대폐내분류솔(Qs/Qt)、양합지수(oxygenation index ,OI)화혈유산(blood lactic acid ,BLA)수평적영향。[방법]선택28례OLV행개흉수술적환자,수궤분장탁저조(P조,우O L V기간무화흡입장탁저)화대조조(C조)。분별우O L V전15 min(T0)、OLV 15 min(T1)、OLV 45 min(T2)추동맥혈여혼합정맥혈행혈기분석급BLA측정,계산Qs/Qt여OI 。[결과]OLV후량조OI균하강,단P조교C조하강완만,차P조T2교T1유소회승;OLV후량조Qs/Qt화BLA균증가,단P조비C조증가교만,차P조T2교T1유소하강。[결론]OLV기간무화흡입장탁저능개선양합,강저폐내분류。
[Objective]To observe the impact of aerosolized penehyclidine hydrochloride inhalation on in-trapulmonary shunt fraction(Qs/Qt) ,oxygenation index(OI) and blood lactic acid(BLA) during one-lung ven-tilation(OLV) .[Methods]Twenty eight patients with OLV undergoing open chest operation were chosen and randomly divided into penehyclidine hydrochloride group(group P ,aerosolized penehyclidine hydrochloride in-halation during OLV) and control group(group C) .Arterial and mixed venous blood were drawn for blood gas analysis and BLA detection 15min before OLV (T0 ) ,15min(T1 ) and 45min after OLV (T2 ) .Qs/Qt and OI were calculated .[Results]After OLV ,OI in two groups decreased ,but that in group P decreased more slowly than that in group C .Compared with T1 ,OI in group P at T2 rose again .After OLV ,Qs/Qt and BLA in two groups increased ,but that in group P increased more slowly than that in group C .Compared with T1 ,Qs/Qt and OI in group P at T2 declined again .[Conclusion]Aerosolized penehyclidine hydrochloride inhalation during OLV can improve oxygenation and decrease intrapulmonary shunt .
