目的:通过彩色超声探讨膈肌厚度与呼吸功能的关系。方法肺叶切除手术患者64例在开胸术中直视下以彩色超声测量膈肌厚度,观测呼吸动力学指标变化。结果以术中彩色超声检测结果分为≤3 mm组7例、4 mm组20例、5 mm组29例和≥6 mm组8例。膈肌厚度4 mm组和5 mm组的各项肺功能指标均优于膈肌≤3 mm组和≥6 mm组,5 mm组的肺功能最佳,而≥6 mm组最差(除SaO2外,均为P﹤0种.05)。结论彩色超声辅助诊断膈肌厚度检测可作为临床对呼吸代偿功能的预测指标,膈肌动力泵的状况可以在一定程度上反映呼吸功能的代偿能力。

Objective To determine the relationship between the thickness of the diaphragm and respiratory function in lobectomy patients using the color Doppler .Methods 64 patients with thoracic lobectomy surgery under direct vision color Doppler ultrasound to measure the thickness of the diaphragm , observe respiratory dynamics indicators'changes .Results Based on intraoperative color ultrasound test results , patients are divided into ≤3 mm groups ( n =7 ) , 4 mm group ( n =20), 5 mm group( n =29) and ≥6 mm group( n =8).Diaphragm thickness of 4 mm group and the 5 mm group's lung function were better than diaphragm ≤3 mm group and ≥6 mm group, the best group of lung function was 5 mm group, and≥6 mm group was the worst (except SaO2, all P <0.05).Conclusion The detection of diaphragm thickness by ultra-sound can be used as a clinical predictor of respiratory compensatory function to a certain extent , the status of diaphragmatic power pump may reflect the compensatory ability of respiratory function .
