目的：探讨观察腹腔镜及开腹阑尾切除术治疗急性坏疽性阑尾炎的临床效果。方法入选2010年3月—2013年1月该院收治的94例住院的急性坏疽性阑尾炎患者，根据家属意愿分为腹腔镜组及开腹组，腹腔镜组59例，开腹组35例。分别于腹腔镜下或直接开腹手术行阑尾切除术。比较2组患者术后并发症发生率总优良率等指标。结果治疗后，开腹组及腹腔镜组的总优良率分别为93.2%及68.6%，差异有统计学意义（χ2=8.20,P<0.05）。结论腹腔镜下行阑尾切除术治疗急性坏疽性阑尾炎临床效果好，并发症发生率低，不易感染，有效缩减住院时间，适于临床广泛应用。
목적：탐토관찰복강경급개복란미절제술치료급성배저성란미염적림상효과。방법입선2010년3월—2013년1월해원수치적94례주원적급성배저성란미염환자，근거가속의원분위복강경조급개복조，복강경조59례，개복조35례。분별우복강경하혹직접개복수술행란미절제술。비교2조환자술후병발증발생솔총우량솔등지표。결과치료후，개복조급복강경조적총우량솔분별위93.2%급68.6%，차이유통계학의의（χ2=8.20,P<0.05）。결론복강경하행란미절제술치료급성배저성란미염림상효과호，병발증발생솔저，불역감염，유효축감주원시간，괄우림상엄범응용。
Objective To observe the clinical effect of laparoscopic and open appendectomy for acute gangrenous appendicitis. Methods 94 cases of acute gangrenous appendicitis patients hospitalized in our hospital from March, 2010 to January, 2013 were divided into laparoscopic group and laparotomy group according to the wishes of the families, 59 cases in the laparoscopic group underwent laparoscopic appendectomy, and 35 cases in laparotomy group underwent open operation for appendectomy. The inci-dence of complications postoperative and total excellent and good rate of two groups were compared. Results After treatment, the total excellent and good rate of laparotomy and laparoscopic group was 93.2% and 68.6%, respectively, the difference was statisti-cally significant (χ2=8.20, P<0.05). Conclusion Laparoscopic appendectomy has better clinical effect on acute gangrenous appendicitis with lower incidence of complications, less infections, shorter length of stay, which is suitable for clinical application extensively.
