上消化道出血多由热入血分、脾不统血等致血溢脉外而成，临床常法以凉血止血、益气摄血为主，疗效多不显著。笔者在临床中发现此类患者除出现出血见证外常兼有舌红、苔黄腻、脉弦等肝阳偏亢之症。基于此，我们从“肝体阴用阳”角度出发，在辨证论治的基础上加入柔肝养血、抑木潜阳等相关中药而获得奇效。本文拟从“肝体阴用阳”角度讨论上消化道出血的治疗体会。
상소화도출혈다유열입혈분、비불통혈등치혈일맥외이성，림상상법이량혈지혈、익기섭혈위주，료효다불현저。필자재림상중발현차류환자제출현출혈견증외상겸유설홍、태황니、맥현등간양편항지증。기우차，아문종“간체음용양”각도출발，재변증론치적기출상가입유간양혈、억목잠양등상관중약이획득기효。본문의종“간체음용양”각도토론상소화도출혈적치료체회。
Upper gastrointestinal tract bleeding was frequently caused by the heat invading into the blood,blood stasis,and spleen failing to control blood, therefore the blood overflow the vessels.In TCM clinics, they were often treated by removing the heat from the blood,arresting bleeding,supplementing Qi and nourishing blood but the effect was poor. In clinical practice the author found that bleeding patients were often accompanied with hyperactivity of liver Yang,such as red tongue, yellow greasy moss and pulse string, etc.Based on this and syn-drome differentiation,and from the view of“Yin nature but Yang function of liver”theory,the prescrption of nour-ishing liver and blood, adding inhibiting wood and strengthening soil and other related Chinese medicine was used and excellent curative effects were obtained.
