目的:探究心衰患者诊断及治疗中血浆脑钠素的应用效果。方法将我院2011年3月~2012年12月接待的79例心衰患者作为研究组,选择同期接待的39例健康人群作为对照组。两组人员皆利用荧光免疫分析技术来对他们的血浆脑钠素水平进行检测,并且利用多普勒超声心电动仪对心衰患者在心室舒张末期的内径(LVEDD)、左心室射血分数(LVEF)及左心室收缩末期内径(LVESD)进行测量,同时采用NYHA(纽约心脏病学会)标准对心功能加以分级。对两组诊断效果进行总结分析及对比。结果研究组在血浆脑钠素浓度上明显高于对照组(P<0.05),有统计学意义;血浆脑钠素浓度在心功能分级上,玉~域级、芋级、郁级之间差异性显著(P<0.05),具有统计学意义;此外,血浆脑钠素的水平与LVEDD、LVEF、LVESD的值有着一定的相关性。结论对于心衰患者的诊断而言,血浆脑钠素在其中的意义十分重大,尤其是其可以灵敏的反映心室功能的相关指标,同时这些指标也能很好的指示治疗效果,值得临床推广及应用。

Objective To assess the usefulness of plasma brain natriuretic peptide for the diagnosis and treatment of patients with heart failure. Methods Seventy-nine patients with heart failure who were admitted to our hospital from March 2011 to December 2012 were enrolled as the study group; and 39 healthy subjects who sought consultations at our hospital in the same period were chosen as the control group. Plasma brain natriuretic peptide levels in both groups were determined using fluorescence immunoassay, and left ventricular end-diastolic diameter (LVEDD), left ventricular ejection fraction (LVEF), and left ventricular end-systolic diameter (LVESD) were measured with Doppler echocardiography. In addition, cardiac function was graded according to the New York Heart Association criteria. Diagnostic results of the two groups were analyzed and compared. Results Plasma brain natriuretic peptide levels were significantly higher in the study group than in the control group (P<0.05). Plasma brain natriuretic peptide levels showed significant difference across cardiac function grades I, II, III, and IV (P<0.05). In addition, plasma brain natriuretic peptide levels exhibited some correlation with LVEDD, LVEF, and LVESD. Conclusion Plasma brain natriuretic peptide is of great significance for the diagnosis of heart failure, particularly because it can sensitively reflect relevant indicators of ventricular function. Meanwhile, these indicators can also be used to assess therapeutic efficacy. Therefore, it is necessary to promote their clinical application.
