目的:研究正常妊娠中晚期孕妇子宫动脉(UtA)的彩色多普勒超声血流阻力的特征。方法选择该院2011年4月—2013年8月检查并于该院正常分娩,经彩色多普勒超声检查465例妊娠20~40周的正常单胎胎儿,测量孕妇双侧子宫动脉(UtA)的血流动力学参数:搏动指数(PI)、阻力指数(RI)、心室收缩期峰值血流速度/心室舒张末期血流速度(S/D)及舒张早期切迹。结果正常中晚孕期孕妇UtA-PI、UtA-RI及UtA-S/D与胎儿孕周呈线性负相关。随孕周增加,其值显著下降。舒张早期切迹随孕周增加而消失。结论彩色多普勒彩超显示子宫血流动力学改变,可作为妊娠期间的综合监测指标。

목적:연구정상임신중만기잉부자궁동맥(UtA)적채색다보륵초성혈류조력적특정。방법선택해원2011년4월—2013년8월검사병우해원정상분면,경채색다보륵초성검사465례임신20~40주적정상단태태인,측량잉부쌍측자궁동맥(UtA)적혈류동역학삼수:박동지수(PI)、조력지수(RI)、심실수축기봉치혈류속도/심실서장말기혈류속도(S/D)급서장조기절적。결과정상중만잉기잉부UtA-PI、UtA-RI급UtA-S/D여태인잉주정선성부상관。수잉주증가,기치현저하강。서장조기절적수잉주증가이소실。결론채색다보륵채초현시자궁혈류동역학개변,가작위임신기간적종합감측지표。
Objective To study the characteristics of uterine artery(UtA) resistance showed by color Doppler ultrasound during the second and third trimesters of normal pregnancy. Methods A total of 465 pregnant women with normal singletons of 20~40 weeks of gestation underwent antenatal color Doppler ultrasound examination and delivered normally in our hospital from April, 2011 to August, 2013 were selected. The hemodynamic parameters of the pregnant women's bilateral uterine artery (UtA) included resis-tance index (RI), pulsatility index (PI), systolic/diastolic ratio(S/D) and early diastolic noctch. Results The UtA-PI value, UtA-RI value and UtA-S/D ratio of normal pregnant women during the second and third trimesters of pregnancy had negative linear corre-lation with the fetal gestational age, and decreased significantly with the increase of the gestational weeks. And the early diastolic notch of uterine artery disappeared with the increase of gestational age. Conclusion The changes of uterine artery hemodynamics showed by color Doppler ultrasound can be taken as the comprehensive monitoring indexes during pregnancy.
