目的:观察硫辛酸注射液治疗百草枯中毒所致肺损害的临床疗效。方法将66例百草枯中毒患者随机分为2组。治疗组33例在常规治疗的基础上加用硫辛酸注射液0.6 g加0.9%氯化钠注射液250 mL,每日2次静脉滴注;对照组33例予一般常规治疗。观察2组用药后第1、7、14 d胸部CT影像学变化、多器官功能障碍综合征( MODS)的发生率、死亡率、动态监测血气分析及急性呼吸窘迫综合征( ARDS)发生率。结果2组治疗7、14 d肺损害总发生率比较差异有统计学意义(P<0.05),治疗组低于对照组。治疗组发生低氧血症、Ⅰ型呼吸衰竭、急性肺损伤(ALI)、ARDS及机械通气少于对照组(P<0.05);治疗组入院后MODS发生率、死亡率低于对照组(P<0.05)。结论硫辛酸注射液对改善百草枯中毒致肺损害、降低死亡率有一定疗效。

목적:관찰류신산주사액치료백초고중독소치폐손해적림상료효。방법장66례백초고중독환자수궤분위2조。치료조33례재상규치료적기출상가용류신산주사액0.6 g가0.9%록화납주사액250 mL,매일2차정맥적주;대조조33례여일반상규치료。관찰2조용약후제1、7、14 d흉부CT영상학변화、다기관공능장애종합정( MODS)적발생솔、사망솔、동태감측혈기분석급급성호흡군박종합정( ARDS)발생솔。결과2조치료7、14 d폐손해총발생솔비교차이유통계학의의(P<0.05),치료조저우대조조。치료조발생저양혈증、Ⅰ형호흡쇠갈、급성폐손상(ALI)、ARDS급궤계통기소우대조조(P<0.05);치료조입원후MODS발생솔、사망솔저우대조조(P<0.05)。결론류신산주사액대개선백초고중독치폐손해、강저사망솔유일정료효。
Objective To observe the effect of thioctic acid injection in the treatment of lung injury induced by paraquat poisoning .Methods 66 cases of paraquat poisoning patients were randomly divided into two groups .33 cases in treatment group received thioctic acid injection of 0.6 g,250 mL 0.9%sodium chloride injection ,twice a day intravenous drip on the basis of conventional treatment;33 cases in control group received conventional treatment . Chest CT imaging changes ,the incidence of MODS and mortality ,dynamic monitoring of the blood gas analysis ,the in-cidence of ARDS were observed after 1,7 and 14 d of medication in two groups .Results The lung damage ( lung door structure disorders,pulmonary fibrosis,etc.) severity of treatment group was obviously lighter than that of the control group after treatment (P<0.05).The incidence of hypoxemia ,Ⅰrespiratory failure,ALI and ARDS,mechan-ical ventilation cases was less than that of the control group (P<0.05).The incidence of MODS and mortality after admission in treatment group was lower than that in the control group (P<0.05).Conclusion Thioctic acid injec-tion has certain curative effect on improving lung injury induced by paraquat poisoning and to reduce the case fatality rate.
