目的:探讨依达拉奉对胸腔镜肺叶切除术患者血清中肺表面活性蛋白A(SP-A)和动脉血气变化的影响。方法将40例行右侧肺叶切除术的肺癌患者随机分为对照组(C组)和依达拉奉组(E组)各20例。E组于诱导后切皮前给予依达拉奉1 mg/kg,30 min内滴完;C组给予等量生理盐水。两组均于切皮前即刻(T0)、单肺通气1 h(T1)和双肺通气后1 h(T2)时抽取动、静脉血,进行动脉血气分析,并测定静脉血血清SP-A的水平。结果两组单肺通气后血清SP-A水平均有明显的升高(P<0.05);与C组相比,E组血清SP-A水平较低(P<0.05),动脉血气也有明显的改善。结论依达拉奉可减少肺叶切除术患者血清 SP-A 水平,在一定程度上能够减轻术中肺部的急性损伤。

목적:탐토의체랍봉대흉강경폐협절제술환자혈청중폐표면활성단백A(SP-A)화동맥혈기변화적영향。방법장40례행우측폐협절제술적폐암환자수궤분위대조조(C조)화의체랍봉조(E조)각20례。E조우유도후절피전급여의체랍봉1 mg/kg,30 min내적완;C조급여등량생리염수。량조균우절피전즉각(T0)、단폐통기1 h(T1)화쌍폐통기후1 h(T2)시추취동、정맥혈,진행동맥혈기분석,병측정정맥혈혈청SP-A적수평。결과량조단폐통기후혈청SP-A수평균유명현적승고(P<0.05);여C조상비,E조혈청SP-A수평교저(P<0.05),동맥혈기야유명현적개선。결론의체랍봉가감소폐협절제술환자혈청 SP-A 수평,재일정정도상능구감경술중폐부적급성손상。
Objective To dynamically assess the implication of Edaravone in serum surfactant protein A (SP-A) concentration and the variation of arterial blood gas during one-lung ventilation (OLV). Methods Forty ASAⅠorⅡ male patients aged 45-65 yr, with body mass index 18-26 kg/m2, without other serious organic disease, undergoing right lung cancer resection, were randomly divided into two groups (n=20 each): group C (control group) and group E (Edaravone group). The patients in group E were given 1 mg/kg of Edaravone (in 100 ml normal saline), which were infused intravenously in 30 min before skin incision, group C received the same volume of normal saline.Arterial and venous blood samples were taken after the induction of Edaravone (T0), at 1 h after OLV (T1) and 1 h after two-lung ventilation (T2). The serum surfactant protein A (SP-A) concentration and the variation of arterial blood gas were determined respectively. Results Compared with T0, the levels of SP-A were increased significantly at T1 in both groups (P<0.05). In group E, the level of Sp-A was lower than that in group C at T1(P<0.05), and the variation of arterial blood gas was also improved. Conclusion Edaravone can reduce the level of SP-A and decrease the injury induced by OLV.
