目的：探讨阿加曲班颈动脉加压滴注治疗急性脑梗死的临床疗效。方法选取我院2012-02-2013-02收治的急性脑梗死患者80例，随机分为观察组和对照组各40例。观察组采用阿加曲班颈动脉加压静滴治疗，观察组采用常规静脉注射等量阿加曲班治疗，比较2组临床疗效，根据治疗前、后2组患者的NIHSS评分评价治疗效果；根据治疗前后60 d mRS评分评价患者的预后情况。结果治疗14 d后，观察组总有效率90.0％，对照组70.0％，观察组临床疗效显著优于对照组，差异有统计学意义（χ2＝5.00，P＝0.019）。2组NIHSS评分较治疗前均显著下降（P＜0.05），且观察组下降更为显著（P＜0.05）。治疗60 d后，2组mRS评分较治疗前显著下降（P＜0.05），且观察组下降更为显著（P＜0.05）。2组患者均未出现明显不良反应。结论阿加曲班颈动脉加压滴注治疗急性脑梗死安全有效，值得临床推广。
목적：탐토아가곡반경동맥가압적주치료급성뇌경사적림상료효。방법선취아원2012-02-2013-02수치적급성뇌경사환자80례，수궤분위관찰조화대조조각40례。관찰조채용아가곡반경동맥가압정적치료，관찰조채용상규정맥주사등량아가곡반치료，비교2조림상료효，근거치료전、후2조환자적NIHSS평분평개치료효과；근거치료전후60 d mRS평분평개환자적예후정황。결과치료14 d후，관찰조총유효솔90.0％，대조조70.0％，관찰조림상료효현저우우대조조，차이유통계학의의（χ2＝5.00，P＝0.019）。2조NIHSS평분교치료전균현저하강（P＜0.05），차관찰조하강경위현저（P＜0.05）。치료60 d후，2조mRS평분교치료전현저하강（P＜0.05），차관찰조하강경위현저（P＜0.05）。2조환자균미출현명현불량반응。결론아가곡반경동맥가압적주치료급성뇌경사안전유효，치득림상추엄。
Objective To analyze the clinical efficacy of carotid artery pressure drip of argatroban on acute cerebral infarc-tion.Methods 80 cases with acute cerebral infarction from February 2012 to February 2013 in our hospital were selected ,ran-domly divided them into control group and observation group ,each of the group with 40 cases. The cases in control group were given argatroban by intravenous infusion ,while those in observation group were given argatroban by carotid artery pressure drip. Clinical dataS of two groups including clinical efficacy ,NIHSS and mRs scores were analyzed.Results The total effective rate of observation group(90.0% ) was much more better that of control group(70.0% );the NIHSS scores and mRs scores of the two groups were significantly lower than those before treatment (P<0.05) ,and it was much lower in observation group.and there was no obvious adverse reactions in both two groups.Conclusion Carotid artery pressure drip of argatroban is efficacious for patients with acute cerebral infarction and is safe. It is worth of clinical promotion.
