【目的】探讨持续气道正压通气(CPAP)对合并阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的2型糖尿病患者胰岛素抵抗(IR)及血清脂联素水平的影响。【方法】选择本院合并OSA HS的2型糖尿病患者48例,通过3个月的CPAP治疗,观察治疗前后反映 IR的稳态模式评估(HOMA )指数及血清脂联素水平的变化。【结果】CPAP治疗3个月后,患者的 HOMA指数较治疗前明显降低(3.58±0.76 vs 1.92±0.69),血清脂联素水平较前升高(5.42±1.35 vs 8.24±1.83),且两组相比较差异均有显著性(均 P <0.01)。【结论】CPAP治疗可显著改善合并OSAHS的2型糖尿病患者的IR及血清脂联素水平。

【목적】탐토지속기도정압통기(CPAP)대합병조새성수면호흡잠정저통기종합정(OSAHS)적2형당뇨병환자이도소저항(IR)급혈청지련소수평적영향。【방법】선택본원합병OSA HS적2형당뇨병환자48례,통과3개월적CPAP치료,관찰치료전후반영 IR적은태모식평고(HOMA )지수급혈청지련소수평적변화。【결과】CPAP치료3개월후,환자적 HOMA지수교치료전명현강저(3.58±0.76 vs 1.92±0.69),혈청지련소수평교전승고(5.42±1.35 vs 8.24±1.83),차량조상비교차이균유현저성(균 P <0.01)。【결론】CPAP치료가현저개선합병OSAHS적2형당뇨병환자적IR급혈청지련소수평。
[Objective]To explore the effect of continuous positive airway pressure (CPAP) on insulin resist-ance(IR) and serum adiponectin level in patients with type 2 diabetes mellitus and obstructive sleep apnea hypopnea syndrome(OSAHS) .[Methods]Forty-eight patients with type 2 diabetes mellitus and OSAHS in our hospital were recruited .The homeostasis model assessment(HOMA) index and serum adiponectin level were measured before and after 3 months of CPAP treatment .[Results]Compared with before treatment ,HOMA index in patients after 3 months of CPAP treatment was significantly decreased (3 .58 ± 0 .76 vs .1 .92 ± 0 .69) ,and serum adiponectin level was significantly increased(5 .42 ± 1 .35 vs .8 .24 ± 1 .83) .There were significant difference between two groups( P <0 .01) .[Conclusion]CPAP treatment can significantly improve IR and serum adiponectin level in pa-tients with type 2 diabetes mellitus and OSAHS .
