目的:探讨可溶性细胞间粘附分子-1(sICAM-1)和降钙素原(PCT)联合检测对新生儿败血症的诊断价值。方法检测47例败血症新生儿和45例健康新生儿血sICAM-1、PCT的表达水平,比较各指标诊断新生儿败血症的灵敏度、特异度、阳性预测值、阴性预测值和准确性。结果败血症组患儿血清sICAM-1及PCT表达水平明显高于对照组(P<0.01);阳性标准分别为:sICAM-1≥300 mg/L,PCT≥2mg/L;血清sICAM-1、PCT联合检测的灵敏度、特异度、阳性预测值、阴性预测值和约登指数均高于血清sICAM-1、PCT单一检测结果。结论血清sICAM-1、PCT联合检测可提高新生儿败血症的诊断灵敏度和特异度,值得临床推广应用。

Objective To investigate the diagnostic value of soluble intercellular adhesion molecule-1 (sICAM-1) combined with procalcitonin (PCT) on detection for neonatal sepsis. Methods The expression level of sICAM-1,PCT in the serum of 47 cases of neonatal sepsis and 45 healthy newborns were detected,and the sensitivity,specificity,positive predictive value, negative predictive value and accuracy of each index for diagnosis of neonatal sepsis were also compared. Results The expression level of PCT and sICAM-1 in group of children with sepsis was significantly higher than that of control group (P<0.01);positive standard were:sICAM-1≥300 mg / L,PCT≥2mg/L;Serum sICAM-1,PCT joint detection sensitivity,specificity,positive predictive value,negative predictive value and Youden index were higher than the single test Results of serum sICAM-1,PCT. Conclusion Serum sICAM-1 combined with PCT improves the diagnostic sensitivity and specificity of neonatal sepsis,and deserves clinical applications.
