目的:比较不同浓度尿激酶( UK)对大鼠腹壁浅动脉岛状皮瓣动脉终末支血栓的溶解效果及临床意义。方法:80只SD大鼠随机分为栓塞组、溶栓组1、溶栓组2和无栓组,每组20只。以血管内缝线技术制备大鼠尾动脉自体血栓模型,设计大鼠腹壁浅动脉岛状皮瓣模型,栓塞组、溶栓组1和溶栓组2建立皮瓣动脉蒂终末支栓塞模型。术后通过微型Doppler仪探测、皮瓣颜色等监测皮瓣血运,以皮瓣成活面积统计皮瓣成活情况。结果:皮瓣存活率:栓塞组为0,溶栓组1为0,溶栓组2为90%,无栓组为100%。4组皮瓣成活面积差异有统计学意义(P<0.05),溶栓组1与溶栓组2相比差异有统计学意义(P<0.01),溶栓组2与栓塞组相比差异有统计学意义( P<0.01)。结论:高浓度UK可以安全、快捷、方便、有效地解除血栓栓子造成的大鼠腹壁浅动脉岛状皮瓣动脉终末支危象。

목적:비교불동농도뇨격매( UK)대대서복벽천동맥도상피판동맥종말지혈전적용해효과급림상의의。방법:80지SD대서수궤분위전새조、용전조1、용전조2화무전조,매조20지。이혈관내봉선기술제비대서미동맥자체혈전모형,설계대서복벽천동맥도상피판모형,전새조、용전조1화용전조2건립피판동맥체종말지전새모형。술후통과미형Doppler의탐측、피판안색등감측피판혈운,이피판성활면적통계피판성활정황。결과:피판존활솔:전새조위0,용전조1위0,용전조2위90%,무전조위100%。4조피판성활면적차이유통계학의의(P<0.05),용전조1여용전조2상비차이유통계학의의(P<0.01),용전조2여전새조상비차이유통계학의의( P<0.01)。결론:고농도UK가이안전、쾌첩、방편、유효지해제혈전전자조성적대서복벽천동맥도상피판동맥종말지위상。
Objective:To compare the efficacy and clinical significance of different concentrations of urokinase for the thrombolysis at the terminal branch of arteries in the abdominal skin flap in rats. Methods:Eighty SD rats were randomly assigned to embolism group ( n=20),thrombolysis group 1 ( n=20),thrombolysis group 2 ( n=20) and nil thrombus group ( n=20) . The intravascular suture technique was used to establish the model of rat-tail arterial thrombosis. The insular skin flap based on the superficial epigastric artery was designed for the 4 groups. The embolism model at the terminal branch of arterial pedicle was established in the embolism group,thrombolysis group 1 and thrombolysis group 2. The blood flow and was monitored by using mini-Doppler ultrasonography and based on the color of the skin flap. The survival rate of skin flap was assessed based on the area that survived. Results: The survival rate of skin flap was 0%, 0%, 90% and 100% in embolism group,thrombolysis group 1, thrombolysis group 2 and nil thrombus group, respectively ( P<0. 05 ) . There was a significant difference between thrombolysis group 1 and thrombolysis group 2, as well as between thrombolysis group 2 and embolism group,in terms of survival rate of skin flap (P<0.01). Conclusion: Thrombolysis using high concentration of urokinase is an effective,safe,prompt and convenient approach for the crisis of embolism at the terminal branch of arteries in the skin flap.
