目的：探讨无创呼吸机治疗慢性阻塞性肺疾病急性加重期伴呼吸衰竭的临床应用价值．方法：回顾性分析2012-01/2014-01我院收治的36例慢性阻塞性肺疾病急性加重期伴呼吸衰竭患者临床资料．结果：经过有效的治疗后，呼吸频率及心率均显著下降，动脉血气指标均显著好转，PaCO2 pH值较治疗前降低，PaO2较治疗前升高，差异具有统计学意义（P＜0．05）；临床总有效率为91．7％．结论：应用无创呼吸机治疗慢性阻塞性肺疾病急性加重期伴呼吸衰竭，可迅速缓解患者的呼吸困难，改善其呼吸肌疲劳，对提高患者的生活能力和生活质量具有重要的临床意义．因其疗效显著、并发症少、操作简单且易于接受，故值得临床广泛应用．

AIM:To explore the clinical value of the noninva-sive ventilator in chronic obstructive pulmonary patients with re-spiratory failure in acute exacerbation period.METHODS:Ret-rospectively analysing the clinical data of 36 cases of chronic ob-structive pulmonary patients admitted from January 2012 to Janu-ary 2014 and accompanied with respiratory failure in acute exacer-bation period.RESULTS:After effective treatment,respiratory frequency and heart rate were significantly decreased,and arterial blood gas index was significantly improved.Compared with the period before effective treatment PaCO2 and the pH value were re-duced,and PaO2 was higher.These differences were statistically significant (P <0.05 );The clinical total effective rate was 91.7%.CONCLUSION:Application of noninvasive ventilator in patients with chronic obstructive pulmonary disease accompan-ying respiratory failure can quickly alleviate breathing difficulties in acute exacerbation period,relieve fatigue of respiratory muscle. And it has important clinical significance in improving the ability and quality of the patient's life.It is worth of applying in the clini-cal practice because of its significant effect,fewer complications, simple operation and easy acceptance.
