目的:探讨sIL-1RⅡ、IL-1β水平及sIL-1RⅡ/IL-1β比值变化在大鼠严重烫伤MODS(多器官功能障碍综合征)中病情严重程度判断及愈后转归预警方面的意义。方法将40只SD大鼠随机分为MODS模型组(严重烫伤+内毒素注射)大鼠30只,假伤对照组10只,分别于制模后24h、72h、120h剪尾取血,采用ELISA法检测两组大鼠各时间点血中sIL-1RⅡ、IL-1β水平,计算sIL-1RⅡ/IL-1β比值,并进行比较。结果(1)模型组大鼠伤后24h、72h、120h血AST、ALT、CK、Crea 水平均明显高于假伤对照组,其中24h时升高最为显著(P<0.01);(2)模型组大鼠伤后24h、72h、120h血IL-1β、sIL-1RⅡ水平明显高于假伤对照组,其中24h时升高最为显著(P<0.01);3、模型组大鼠伤后24h、72h、120h 血sIL-1RⅡ/IL-1β比值比值明显低于假伤对照组,其中24h时降低最为显著(P<0.01)。结论血sIL-1RⅡ/IL-1β比值的变化反映了MODS中机体免疫调节的情况,其比值的高低及动态变化对判断MODS器官功能不全程度及预后转归预警具有重要意义。

목적:탐토sIL-1RⅡ、IL-1β수평급sIL-1RⅡ/IL-1β비치변화재대서엄중탕상MODS(다기관공능장애종합정)중병정엄중정도판단급유후전귀예경방면적의의。방법장40지SD대서수궤분위MODS모형조(엄중탕상+내독소주사)대서30지,가상대조조10지,분별우제모후24h、72h、120h전미취혈,채용ELISA법검측량조대서각시간점혈중sIL-1RⅡ、IL-1β수평,계산sIL-1RⅡ/IL-1β비치,병진행비교。결과(1)모형조대서상후24h、72h、120h혈AST、ALT、CK、Crea 수평균명현고우가상대조조,기중24h시승고최위현저(P<0.01);(2)모형조대서상후24h、72h、120h혈IL-1β、sIL-1RⅡ수평명현고우가상대조조,기중24h시승고최위현저(P<0.01);3、모형조대서상후24h、72h、120h 혈sIL-1RⅡ/IL-1β비치비치명현저우가상대조조,기중24h시강저최위현저(P<0.01)。결론혈sIL-1RⅡ/IL-1β비치적변화반영료MODS중궤체면역조절적정황,기비치적고저급동태변화대판단MODS기관공능불전정도급예후전귀예경구유중요의의。
Objective To investigate the clinical significance of soluble interleukin-1 receptorⅡ (sIL-1RⅡ), intedeukin-1β(IL-1β) and the ratio of sIL-1RⅡ/IL-1β in restoration warning and status monitoring of multiple organ dysfunction syndrome (MODS). Methods According to the random number table, fourty SD rats were divided into the MODS model group (severe scald-ed and endotoxin injection, n=30) and the sham scalded models group (n=10). Blood samples were collected by tail trimming at three time points after scald, 24h, 72h and 120h. The levels of sIL-1RⅡand IL-1β in rats plasma were detected by enzyme-linked immunoabsorbent assay (ELISA), and the ratios of sIL-1RⅡ/IL-1βwere calculated. At the same time, automatic biochemi-cal analyzer was used to detect the AST, ALT, CK and Crea levels. Results At 24h, 72h and 120h after the model group were in-jured, levels of AST, ALT, CK and Crea in model group rats blood were significantly higher than control group, and the most sig-nificant rising appeared at 24h (P<0.01). At 24h, 72h and 120h after the model group were injured, levels of IL-1βand sIL-1RⅡin model group rats blood were significantly higher than control group, and the most significant rising appeared at 24h (P<0.01). At 24h, 72h and 120h after the model group were injured, ratio of sIL-1RⅡ/IL-1β in model group rats blood were significantly low-er than control group, and the most significant reducing appeared at 24h (P<0.01). Conclusion The ratio of sIL-1RⅡ/IL-1βcan reflect the immune regulation status in severely scalded rats with MODS. The ratios degree and dynamic variation play an impor-tant role in warning restoration and monitoring the status of MODS.
