目的：探讨不同性状乳头溢液与乳管内不同病变之间的相关性。方法对2007年3月~2012年3月在我院诊治的91例乳头溢液患者临床资料进行回顾性研究。结果91例乳头溢液患者中发现乳管内非占位性病变37例（40.7%）；占位性病变54例（59.3%），其中乳腺癌5例（5.5%）。与双侧、多孔、非血性乳头溢液患者相比，单侧、单孔、血性溢液患者更具有临床意义（P＜0.05）；与非咖啡样血性溢液相比，咖啡样血性溢液更具有临床意义（P＜0.01）。结论乳头溢液是乳管内病变的早期表现，也是早期乳腺癌的重要征象。不同性状乳头溢液与乳管内不同性质的疾病之间密切相关，应高度重视单侧、单孔、血性溢液，尤其是咖啡样乳头溢液。
목적：탐토불동성상유두일액여유관내불동병변지간적상관성。방법대2007년3월~2012년3월재아원진치적91례유두일액환자림상자료진행회고성연구。결과91례유두일액환자중발현유관내비점위성병변37례（40.7%）；점위성병변54례（59.3%），기중유선암5례（5.5%）。여쌍측、다공、비혈성유두일액환자상비，단측、단공、혈성일액환자경구유림상의의（P＜0.05）；여비가배양혈성일액상비，가배양혈성일액경구유림상의의（P＜0.01）。결론유두일액시유관내병변적조기표현，야시조기유선암적중요정상。불동성상유두일액여유관내불동성질적질병지간밀절상관，응고도중시단측、단공、혈성일액，우기시가배양유두일액。
Objective To explore the correlation of different characteristics of nipple discharge and intraductal lesions. Methods 91 cases with nipple discharge were admitted from Mar 2007 to Mar 2012 in our hospital. Their documents were an-alyzed retrospectively. Results 91 cases were successfully diagnozed and treated with the help of fiberoptic ductoscopy. 37 cases (40.7%)were diagnozed as non-protrusion lesions. 54 cases (59.3%)were diagnozed as protrusion lesions and performed operation. 5 cases (5.5%)of early breast cancer were confirmed by postoperative pathology. In comparison with bilateral, porous and no-bloody nipple discharge, there was more clinical significance in unilateral, single duct and bloody nipple discharge (P<0.05). Compared with non coffee like bloody discharge, there was more clinical significance in coffee nipple discharge(P<0.01). Conclusion Nipple discharge is an early manifestation of breast intraductal lesions, and also is the important sign of early breast cancer. There should be a closely relationship between different nipple discharge and breast intraductal lesions. More attention should be paid to the signs of unilateral, single duct and bloody nipple discharge, especially coffee nipple discharge.
