目的:探讨术前血浆瘦素水平与胃癌患者BMI及术后病理分期相关性。方法收集明确诊断为原发性胃癌患者130例(胃癌组)及40名明确排除胃癌健康成人(对照组)的临床资料。比较两组人群营养状况指标、BMI及血浆瘦素水平。采用Spearman秩相关分析血浆瘦素水平与胃癌患者BMI和术后病理分期相关性。结果胃癌患者BMI水平与对照组相当(P"0.05),但胃癌患者营养指标如白蛋白、转铁蛋白、前白蛋白、总胆固醇及血红蛋白水平均较低,与对照组比较差异有统计学意义(P<0.05),而血浆瘦素水平在胃癌组则高于对照组(P<0.05)。Spearman秩相关分析提示,胃癌患者术前血浆瘦素水平与BMI呈负相关(r=-0.536,P<0.05),与术后病理分期呈正相关(r=0.558,P<0.05)。结论术前血浆瘦素水平与胃癌患者术前全身营养状况及术后病理分期具有相关性,检测血浆瘦素水平对预后评估具有一定临床价值。

Objective Currentstudywasconductedtoinvestigatethecorrelationbetweenpreopera-tive serum level of leptin and pathological stage and body mass index (BMI)in patients with gastric cancer.Methods Onehundredandthirtypatientsdiagnosedasprimarygastriccancerand40healthysubjectswere assigned into the gastric cancer and control groups. Nutritional condition,BMI and serum level of leptin were statistically compared between two groups. Spearman rank correlation test was performed to evaluate the correla-tion between preoperative serum level of leptin and BMI and postoperative pathological stage in patients with gastriccancer.Results BMIwassimilarbetweentwogroups.However,nutritionalindexessuchasalbumin, transferrin,prealbumin,cholesterol and hemoglobin levels in gastric cancer group were significantly lower than those of the control group (all P<0.05 ). Serum level of leptin was significantly higher in gastric cancer group compared with that in the control group (P<0.05 ). Spearman rank correlation test revealed that serum level of leptin was negatively correlated with BMI with a correlation coefficient of -0.536 (P<0.05 ),whereas was positively correlated with pathological stage with a correlation coefficient of 0.558 (P<0.05). Conclusion Preoperativeserumlevelofleptiniscorrelatedwithpathologicalstageandnutritionalstatusofpatientswithgas-tric cancer. Preoperative measurement of serum leptin level is help to evaluate the prognosis of gastric cancer.
