目的:探讨住院2型糖尿病患者糖尿病周围神经病变(DPN)的相关因素。方法收集219例2型糖尿病住院患者的临床资料,根据是否合并DPN,分为DPN组(153例)及非DPN组(66例),比较两组患者的临床资料,采用Logistic回归分析探讨住院2型糖尿病患者DPN 的相关因素。结果与非DPN组相比,DPN组年龄高、糖尿病病程长(P<0.01),糖化血红蛋白(HbA1 c )、空腹血糖、餐后2 h血糖、尿微量白蛋白排泄率(UAER)水平及合并糖尿病视网膜病变(DR)比例高(P<0.05或0.01)。Logistic回归分析显示DPN与年龄、HbA1 c、UAER及合并DR有关(OR值分别为1.071、1.315、1.039、2.255)。结论年龄、HbA1 c、UAER及合并DR是住院2型糖尿病患者DPN的重要相关因素,高龄、HbA1 c高、UAER高及合并 DR的住院2型糖尿病患者可能更易发生DPN。

Objective Toexplorerelevantfactorsofdiabeticperipheralneuropathy(DPN)inhospi-talizedpatientswithtype2diabetesmellitus.Methods Atotalof219patientswithtype2diabeteswerere-cruited into this study and divided into the DPN (n=1 53)and non-DPN groups (n=66)according to periph-eral neuropathy. Basic clinical data were statistically compared between two groups. Logistic regression analysis wasadoptedtoidentifyrelevantfactorsofDPN.Results Comparedwiththenon-DPNgroup,patientsinthe DPN group were older and had a longer duration of diabetes (both P<0.01 ). The levels of glycosylated hemo-globin (HbA1 c ),fasting blood glucose (FPG),2 hour postprandial blood glucose (2hPG),urinary albumin excretory rate (UAER)and the incidence of diabetic retinopathy (DR)in DPN patients were significantly higher than those in non-DNP counterparts (P<0.05 or P<0.01 ). Logistic regression analysis revealed that age,HbA1 c ,UAER and complication with DR were significantly correlated with DPN (OR were 1.071 , 1.315,1.039and2.255,respectively).Conclusions Age,HbA1c,UAERandcomplicationwithDRare pivotal relevant factors of DPN in hospitalized patients with type 2 diabetes mellitus. Diabetic patients who are older,have higher levels of HbA1 c and UAER and are complicated with DR have a high incidence of DPN.
