目的:探讨呼出气一氧化氮(FeNO)检测用于预测布地奈德治疗慢性咳嗽疗效的临床价值。方法选取支气管激发试验或舒张试验阴性、诱导痰嗜酸性粒细胞小于0.03、FeNO水平升高的慢性咳嗽患者共60例,将其随机分为治疗组和对照组各30例。对照组采用罗红霉素、氨溴索治疗。治疗组加用布地奈德吸入治疗,每次2 mg,每日2次。结果治疗1周后,治疗组的临床缓解率及总有效率均高于对照组(P均<0.01)。治疗组治疗后FeNO水平比治疗前明显下降(P<0.01),而对照组治疗前、后FeNO水平变化不明显(P>0.05)。结论 FeNO检测对于慢性咳嗽患者选择用药有指导作用,布地奈德治疗FeNO升高的慢性咳嗽患者的疗效确切。

목적:탐토호출기일양화담(FeNO)검측용우예측포지내덕치료만성해수료효적림상개치。방법선취지기관격발시험혹서장시험음성、유도담기산성립세포소우0.03、FeNO수평승고적만성해수환자공60례,장기수궤분위치료조화대조조각30례。대조조채용라홍매소、안추색치료。치료조가용포지내덕흡입치료,매차2 mg,매일2차。결과치료1주후,치료조적림상완해솔급총유효솔균고우대조조(P균<0.01)。치료조치료후FeNO수평비치료전명현하강(P<0.01),이대조조치료전、후FeNO수평변화불명현(P>0.05)。결론 FeNO검측대우만성해수환자선택용약유지도작용,포지내덕치료FeNO승고적만성해수환자적료효학절。
Objective Toevaluatetheclinicalvalueoffractionalexhalednitricoxide(FeNO)in predictingtheefficacyofpulmicortrespulesintreatmentofchroniccough.Methods Sixtychroniccoughpa-tients with negative results in bronchial provocation and dilation tests,induced sputum eosinophil count <3%and elevated levels of FeNO were randomly divided into the treatment (n=30)and control groups (n=30).In the control group,roxithromycin and mucosolvan were used for treatment. In the treatment group,pulmicort respulesinhalationwasperformedadditionally,2mgforeachtestfortwicedaily.Results Afteroneweek treatment,the remission rate and overall efficacy in the treatment group were significantly higher compared with those in the control group (both P<0.01 ). FeNO level in the treatment group was significantly decreased (P<0.01)whereastheFeNOlevelinthecontrolgroupdidnotdiffer(P>0.05).Conclusion FeNOplaysa role in selecting medication therapy for patients with chronic cough. Pulmicort respules inhalation is effective in treatment of chronic cough with elevated levels of FeNO.
