目的探讨全胸腔镜下肺癌根治术对早期非小细胞肺癌的可行性，评价其临床应用价值。方法回顾性分析术后病理分期为Ⅰ期、Ⅱ期的非小细胞肺癌患者68例，分别采用全胸腔镜手术（VATS 组）治疗，传统开胸（TOS组）治疗，比较两组患者的术中术后各项指标。结果两组在手术时间、术中出血量、清扫纵隔淋巴结数目、并发症发生率对比差异无统计学意义(P＞0.05)。术后胸腔引流量、早期下床活动时间、术后住院天数、术后疼痛程度VATS组均低于TOS组(P＜0.05)。结论 VATS 肺癌根治术治疗早期肺癌安全可行，且术后恢复快、疼痛程度轻。
목적탐토전흉강경하폐암근치술대조기비소세포폐암적가행성，평개기림상응용개치。방법회고성분석술후병리분기위Ⅰ기、Ⅱ기적비소세포폐암환자68례，분별채용전흉강경수술（VATS 조）치료，전통개흉（TOS조）치료，비교량조환자적술중술후각항지표。결과량조재수술시간、술중출혈량、청소종격림파결수목、병발증발생솔대비차이무통계학의의(P＞0.05)。술후흉강인류량、조기하상활동시간、술후주원천수、술후동통정도VATS조균저우TOS조(P＜0.05)。결론 VATS 폐암근치술치료조기폐암안전가행，차술후회복쾌、동통정도경。
Objective To investigate the feasibility of full thoracoscopic lung resection for early non-small cell lung cancer, and evaluate its clinical value. Methods 68 patients with non-small cell lung cancer in the stage ofⅠ,Ⅱwere retrospectively analyzed. Among them, VATS group was treated with full-assisted thoracic surgery, and TOS group was treated with traditional thoracotomy surgery. The indicators in operation were compared between two groups. Results There were no statistically significant difference about the operation time, the blood loss, intraoperative lymph node yield and the incidence of complications between two groups. (P>0.05). But compared with TOS, VATS was associated with significantly shortened postoperative hospital stay and lighter pain (P<0.05). Conclusion VATS lung cancer resection in the treatment of early stage lung cancer has the advantages of feasible and safe, quick recovery, lighter pain.
