目的:探讨N-末端脑钠素原在小儿支原体肺炎并心肌损害中的水平改变意义。方法选择我院支原体肺炎合并心肌损害患儿共64例,根据心功能评定,上述患儿分为无心力衰竭组、轻度心力衰竭组、中度心力衰竭组和重度心力衰竭组。测定上述患儿的心肌肌钙蛋白、肌酸激酶同工酶、超敏C反应蛋白和N-末端脑钠素原水平。结果轻度心力衰竭组的心肌肌钙蛋白、肌酸激酶同工酶、超敏C反应蛋白和N-末端脑钠素原水平分别高于无心力衰竭组患者,差异有统计学意义(P<0.05);中度心力衰竭组患者的心肌肌钙蛋白、肌酸激酶同工酶、超敏C反应蛋白和N-末端脑钠素原水平分别高于轻度心力衰竭组患者,差异有统计学意义(P<0.05);重度心力衰竭组患者的心肌肌钙蛋白、肌酸激酶同工酶、超敏C反应蛋白和N-末端脑钠素原水平分别高于中度心力衰竭组患者,差异有统计学意义(P<0.05)。结论 N-末端脑钠素原水平改变有助于评估小儿支原体肺炎并心肌损害严重程度,有助于评估患儿预后,具有重要的临床意义。

Objective To explore the chang significance of level of N- terminal pro brain natriuretic peptide in children with mycoplasma pneumonia and myocardial damage. Methods 64 children with mycoplasma pneumonia and myocardial damage were selected and divided into non heart failure group, mild heart failure group,moderate group and severe heart failure group and heart failure ccording to the assessment of cardiac function.Myocardial troponin,creatine kinase isoenzyme,high sensitive C reactive protein and N- terminal pro brain natriuretic peptide levels were detected in children. Results The level of myocardial troponin,creatine kinase isoenzyme,high sensitive C reactive protein and N-terminal pro brain natriuretic peptide in the mild heart failure group were higher than those in non heart failure group,the difference was statistically significant (P<0.05);The level of myocardial troponin,creatine kinase isoenzyme, high sensitive C reactive protein and N- terminal pro brain natriuretic peptide t in the moderate group heart failure group were higher than those in mild heart failure group,the difference was statistically significant (P<0.05);The level of myocardial troponin,creatine kinase isoenzyme,high sensitive C reactive protein and N-terminal pro brain natriuretic peptide t in the sever heart failure group were higher than those in moderate heart failure group,the difference was statistically significant(P < 0.05). Conclusion N- terminal pro brain natriuretic peptide levels contribute to the assessment of the severity of myocardial damage in children with mycoplasma pneumonia a,and contribute to the assessment of prognosis,which has important clinical significance.
