目的:探讨老年2型糖尿病患者骨质疏松症(OP)的危险因素。方法选取我院老年病科2012年12月~2013年12月门诊及住院的老年2型糖尿病患者192例,分为OP组(A组)和非OP组的(B组)。收集患者的一般临床资料及实验室检查结果,运用t检验对两组一般资料进行比较,并对相关危险因素进行Logistic回归分析。结果(1)两组HbA1C(糖化血红蛋白A1C)、FINS(空腹胰岛素)、FBG(空腹血糖)、BMI(体重指数)差异有统计学意义(P<0.05);Ca2+(血钙)、P(血磷)比较差异无统计学意义(P>0.05)。(2) HbA1C、FINS水平、病程、BMI是老年2型糖尿病患者OP的独立危险因素(P<0.05)。结论积极控制血糖、保护胰岛功能、维持理想体重,对于老年2型糖尿病患者预防骨质疏松症具有重要参考价值。

목적:탐토노년2형당뇨병환자골질소송증(OP)적위험인소。방법선취아원노년병과2012년12월~2013년12월문진급주원적노년2형당뇨병환자192례,분위OP조(A조)화비OP조적(B조)。수집환자적일반림상자료급실험실검사결과,운용t검험대량조일반자료진행비교,병대상관위험인소진행Logistic회귀분석。결과(1)량조HbA1C(당화혈홍단백A1C)、FINS(공복이도소)、FBG(공복혈당)、BMI(체중지수)차이유통계학의의(P<0.05);Ca2+(혈개)、P(혈린)비교차이무통계학의의(P>0.05)。(2) HbA1C、FINS수평、병정、BMI시노년2형당뇨병환자OP적독립위험인소(P<0.05)。결론적겁공제혈당、보호이도공능、유지이상체중,대우노년2형당뇨병환자예방골질소송증구유중요삼고개치。
Objective To investigate the risk factors of osteoporosis (OP) in elderly patients with type 2 diabetes. Methods 192 elderly outpatients and inpatients with type 2 diabetes treated in the geriatric department of our hospital from December 2012 to December 2013 were selected and divided into the OP group (group A) and the non-OP group (group B). The patients' ordinary clinical data and laboratory inspection results were collected. The ordinary data of the two groups were compared using t test and relevant risk factors were analyzed using Logistic regression analysis. Results (1) The two groups were statistically different in HbA1C (glycosylated hemoglobin A1C), FINS(fasting insulin), FBG (fasting blood glucose) and BMI (body mass index)(P < 0.05). (2) HbA1C, FINS level, course of disease and BMI were independent risk factors of OP in elderly patients with type 2 diabetes(P < 0.05). Conclusion Active control of blood sugar, protection of insulin function and maintenance of ideal body weight are of important reference value for the prevention of osteoporosis in elderly patients with type 2 diabetes.
