目的：探讨家庭医生责任制中居家保健服务在社区慢性病防治中的效果。方法分析我社区慢性病患者200例资料，根据家庭医生责任制中居家保健服务实施前后进行评分。结果居家保健服务实施后自我效能评分、治疗依从性均明显优于居家保健服务实施前，差异均有统计学意义（P＜0.05）。结论家庭医生责任制中居家保健服务在社区慢性病患者中应用后可以明显提高患者对于慢性疾病的认知程度，提高患者治疗的依从性。

Objective To approach efficacy of community chronic disease by home care service in responsibility system of family doctors. Methods To analyze 200 data of community chronic disease patients in our community. To score of community chronic disease by home care service in responsibility system of family doctors before and after implement. Results The treatment compliance scores and self-efficacy after implement of home care service were better than before implement of home care service,the difference was statistical significance(P<0.05). Conclusion The cognitive level of chronic disease is improved by home care service in responsibility system of family doctors, compliance of treatment is improved.
