目的:探讨血清sCD40L结合BNP对急性心肌梗死近期预后的预测价值。方法选择150例患者,分为3组,各50例,其中SCD40L升高者50例(A组),BNP升高者50例(B组),SCD40L和BNP均升高者50例(C组),随访1个月,比较3组发生死亡、恶性心律失常、再梗、休克等事件的情况。结果24h、7d和1个月内C组发生死亡、恶性心律失常及休克的比率高于A组和B组(P<0.05),24h 内A组发生死亡、恶性心律失常及休克的比率高于B组(P<0.05),7d和1个月内B组发生死亡、恶性心律失常、再梗的比率高于A组(P<0.05)。结论 sCD40L升高可预测心急梗死急性期发生死亡、恶性心律失常、再梗及休克的几率升高,而BNP水平升高则对亚急性其发生死亡、恶性心律失常、再梗及休克的几率具有预测价值。

목적:탐토혈청sCD40L결합BNP대급성심기경사근기예후적예측개치。방법선택150례환자,분위3조,각50례,기중SCD40L승고자50례(A조),BNP승고자50례(B조),SCD40L화BNP균승고자50례(C조),수방1개월,비교3조발생사망、악성심률실상、재경、휴극등사건적정황。결과24h、7d화1개월내C조발생사망、악성심률실상급휴극적비솔고우A조화B조(P<0.05),24h 내A조발생사망、악성심률실상급휴극적비솔고우B조(P<0.05),7d화1개월내B조발생사망、악성심률실상、재경적비솔고우A조(P<0.05)。결론 sCD40L승고가예측심급경사급성기발생사망、악성심률실상、재경급휴극적궤솔승고,이BNP수평승고칙대아급성기발생사망、악성심률실상、재경급휴극적궤솔구유예측개치。
ObjectiveTo investigate the predictive value of BNP and serum sCD40L for prognosis of acute myocardial infarction.Methods 150 patients were divided into three groups, each 50 cases, sCD40L increased in 50 cases (A group), BNP increased in 50 cases (group B), sCD40L and BNP were both increased in 50 cases (group C), all cases were followed up by 1 month, then the death rate, malignant arrhythmia, reinfarction acute myocardial infarction, shock of the event were compared with the three groups. ResultsAfter 24h, 7 days and 1 month, the death occurred, malignant arrhythmias and shock the ratio in group C were higher than group A and group B (P<0.05), 24h death occurs, malignant arrhythmias and shock the ratio within Group A were higher than group B(P<0.05), 7 days and 1 month with death occurred, malignant arrhythmia, reinfarction ratio in group B were higher than group A (P<0.05). Conclusion sCD40L elevate with acute phase of myocardial infarction, it may predict the acute occurrence of death, malignant arrhythmia, reinfarction and the chance of shock, while BNP levels increases, it may predict the subacute occurrence of death, malignant arrhythmia, reinfarction and the chance of shock.
