目的:评价晚期非小细胞肺癌( NSCLC)患者放射性治疗疗效及副作用与XRCC1 Codon399单核苷酸多态性的相关性。方法经皮肺穿刺活检病理确诊为鳞癌、腺癌、腺鳞癌或大细胞癌患者60例,以xTAG液相芯片技术检测XRCC1 Codon399的3种基因型。随访至放疗结束后3个月,评价放疗效果和放射性损伤与不同基因型的关系。结果各组在放疗效果方面未发现显著差异。 Arg/Gln杂合子组患者发生急性放射性肺损伤的比例高于两组纯合子( P<0.05), Arg/Gln和 Gln/Gln组患者在消化道损伤方面要高于Arg/Arg纯合子组(P<0.05)。在皮肤、食管损伤以及白细胞、血小板等指标未发现显著差异(P>0.05)。结论 XRCC1 Codon399单核苷酸多态性与NSCLC放疗敏感性无关,但与晚期NSCLC患者肺部及消化道的急性放射性损伤有关,有望成为放射治疗副作用的预测因子之一。

목적:평개만기비소세포폐암( NSCLC)환자방사성치료료효급부작용여XRCC1 Codon399단핵감산다태성적상관성。방법경피폐천자활검병리학진위린암、선암、선린암혹대세포암환자60례,이xTAG액상심편기술검측XRCC1 Codon399적3충기인형。수방지방료결속후3개월,평개방료효과화방사성손상여불동기인형적관계。결과각조재방료효과방면미발현현저차이。 Arg/Gln잡합자조환자발생급성방사성폐손상적비례고우량조순합자( P<0.05), Arg/Gln화 Gln/Gln조환자재소화도손상방면요고우Arg/Arg순합자조(P<0.05)。재피부、식관손상이급백세포、혈소판등지표미발현현저차이(P>0.05)。결론 XRCC1 Codon399단핵감산다태성여NSCLC방료민감성무관,단여만기NSCLC환자폐부급소화도적급성방사성손상유관,유망성위방사치료부작용적예측인자지일。
Objective To investigate the association between radiation effect and injury in normal tissues of patients with non-small cell lung cancer (NSCLC) and single nucleotide polymorphism of XRCC1 Codon399. Meth-ods 60 patients with pathologically confirmed squamous carcinoma, adenocarcinoma, adenosquamous carcinoma or large cell carcinoma were respectively analyzed. The genotype of XRCC1 Codon399 was detected by xTAG liquid chip. The late irradiation-induced injury was evaluated 3 months after radiotherapy. Results There was no signifi-cant difference involved radio-sensitivity. The patients with Arg/Gln heterozygote had a higher rate of acute radiation-induced pulmonary injury than the others ( P<0. 05 ) , meanwhile the patients with Arg/Gln heterozygote and Gln/Gln homozygote were easier to injury mucous membrane of gastrointestinal tract (P<0. 05). There was no significant difference among the three genotypes involved cute irradiation-induced injury with skin, pharynx and esophagus, leu-kocyte blood platelet (P>0. 05). Conclusion The single nucleotide polymorphism of XRCC1 Codon399 has noth-ing to do with radio-sensitivity, and it is associated with acute irradiation-induced injury wit pulmonary and gastroin-testinal tract in patients with advanced NSCLC. It is expected to be one of the predictive factors with the side effect of radiotherapy.
