目的：研究老年脑梗死患者血同型半胱氨酸(Hcy)与颈动脉斑块的相互关系，探讨二者对脑梗死事件预测的临床意义。方法对150例脑梗死(病例组)患者与120例健康体检者(对照组)空腹抽取静脉血行血Hcy测定。对所有入选者采用彩色多普勒超声进行颈部血管检测，测量颈动脉粥样斑块并分析其稳定性。结果脑梗死组Hcy明显高于对照组，两组比较有显著性差异(P<0.05)。不稳定斑块组血Hcy高于稳定斑块组血Hcy，两组之间有统计学差异(P<0.05)。脑梗死组重度脑梗死患者Hcy明显高于轻、中度脑梗死患者。结论血清Hcy水平增高与脑梗死患者的颈动脉斑块发生和严重程度密切相关，监测血Hcy水平变化有助于脑梗死早期干预及治疗。
목적：연구노년뇌경사환자혈동형반광안산(Hcy)여경동맥반괴적상호관계，탐토이자대뇌경사사건예측적림상의의。방법대150례뇌경사(병례조)환자여120례건강체검자(대조조)공복추취정맥혈행혈Hcy측정。대소유입선자채용채색다보륵초성진행경부혈관검측，측량경동맥죽양반괴병분석기은정성。결과뇌경사조Hcy명현고우대조조，량조비교유현저성차이(P<0.05)。불은정반괴조혈Hcy고우은정반괴조혈Hcy，량조지간유통계학차이(P<0.05)。뇌경사조중도뇌경사환자Hcy명현고우경、중도뇌경사환자。결론혈청Hcy수평증고여뇌경사환자적경동맥반괴발생화엄중정도밀절상관，감측혈Hcy수평변화유조우뇌경사조기간예급치료。
Objective To explore the relationship between senile cerebral infarction patients and blood homocysteine,carotid artery plaque and to investigate the clinical significance of the two prediction of cerebral infarction events. Methods A total of 150 patients with cerebral infarction (case group) and 120 healthy persons (control group) were checked-up hollow venous blood to determine the level of blood homocysteine. The list of all were detected by color doppler ultrasound in the neck, to observe the measurement of blood vessels detecting neck atheromatous plaque and analyze the stability. Results The level of blood homocysteine in the case group was obviously higher than that of the control group,the difference between the two groups was statistically significant(P<0.05). The level of blood homocys-teine in unstable carotid atherosclerotic plaque groups than that of the stable group,between the two groups as statisti-cally significant(P<0.05). The level of blood homocysteine in the severe cerebral infarction group was obviously higher than that of the mild and moderate group. Conclusion There is a real relationship between the level of the blood homo-cysteine and carotid plaques in patients with cerebral infarction and monitoring of the change of Hcy levels is helpful in the early intervention and treatment of cerebral infarction.
