目的：分析应用经尿道绿激光行膀胱部分切除术治疗膀胱肿瘤的手术方法、安全性与疗效。方法2011年7月至2012年10月在本院应用经尿道绿激光膀胱部分切除术治疗的24例膀胱肿瘤患者，共切除肿瘤29个。术中以绿激光沿肿瘤周围全层切开并在肌层与外层结缔组织间剥离膀胱壁，将肿瘤及其基底部膀胱壁整块切除。记录手术时间、术中出血情况、术中术后并发症、肿瘤病理结果及术后随访结果。结果24例患者均可耐受手术，本组患者手术时间10～23分钟，平均（14．2±4．3）分钟；术中出血量极少或几乎不出血，无闭孔神经反射，术后亦无继发出血，术后留置导管1～3天，无需膀胱冲洗。术后病理示T1期18例，T2期6例，随访12个月，复发1例，无原位复发。结论经尿道绿激光膀胱部分切除术治疗膀胱肿瘤，疗效确切，安全可靠。
목적：분석응용경뇨도록격광행방광부분절제술치료방광종류적수술방법、안전성여료효。방법2011년7월지2012년10월재본원응용경뇨도록격광방광부분절제술치료적24례방광종류환자，공절제종류29개。술중이록격광연종류주위전층절개병재기층여외층결체조직간박리방광벽，장종류급기기저부방광벽정괴절제。기록수술시간、술중출혈정황、술중술후병발증、종류병리결과급술후수방결과。결과24례환자균가내수수술，본조환자수술시간10～23분종，평균（14．2±4．3）분종；술중출혈량겁소혹궤호불출혈，무폐공신경반사，술후역무계발출혈，술후류치도관1～3천，무수방광충세。술후병리시T1기18례，T2기6례，수방12개월，복발1례，무원위복발。결론경뇨도록격광방광부분절제술치료방광종류，료효학절，안전가고。
Objective To analyze the safety and efficacy of partial cystectomy in treatment of bladder tumor by greenlight laser .Methods A total of 24 patients with 29 bladder tumors underwent greenlight laser via tran-surethral from July 2011 to October 2012.In the operation , the tumors were vaporized and cut with the whole wall of bladder, and dissected from connective tissue for pathology inspection .Operation time, blood loss, perio-perative complications and pathology staging of tumors were recorded and postoperative follow -ups were per-formed.Results All the patients were well tolerated in the operation .The mean operative time was 10~23 min (average,14.2 ±4.3min).Blood loss in the operation was minimal.No patient experienced an obturator nerve reflection or massive hemorrhage after the operation .Catheter-indwelling was needed for 1~3 days and no blad-der washing was necessary .The pathology staging of the tumors were as follows: T1 stage 18 cases, T2 stage 6 cases.Tumor recurrence was found in 1 case, no one had recurrence in situ .Conclusions Transurethral partial cystectomy by greenlight laser is a safe and effective method in the treatment of bladder tumor .
