目的:探讨微柱凝胶卡式法对新生儿溶血病(HDN)的诊断价值。方法:选取2012年7月-2013年7月本院儿科收治的疑似ABO母婴血型不合新生儿溶血病的278例新生儿血标本,分别用微柱凝胶卡式法和传统试管法进行直接抗人球蛋白试验、间接抗人球蛋白试验和放散试验,并对检测结果进行比较。结果:278例中临床确诊263例。微柱凝胶卡式法的直接抗人球蛋白试验阳性率76.4%明显高于试管法的59.3%,抗体游离试验阳性率90.5%明显高于试管法的66.2%,抗体放散试验阳性率99.2%明显高于试管法的79.1%,差异均有统计学意义(P<0.001)。结论:微柱凝胶卡式法对于辅助诊断HDN优于传统试管法,操作简单,耗时短,结果易观察,值得在临床上推广。

목적:탐토미주응효잡식법대신생인용혈병(HDN)적진단개치。방법:선취2012년7월-2013년7월본원인과수치적의사ABO모영혈형불합신생인용혈병적278례신생인혈표본,분별용미주응효잡식법화전통시관법진행직접항인구단백시험、간접항인구단백시험화방산시험,병대검측결과진행비교。결과:278례중림상학진263례。미주응효잡식법적직접항인구단백시험양성솔76.4%명현고우시관법적59.3%,항체유리시험양성솔90.5%명현고우시관법적66.2%,항체방산시험양성솔99.2%명현고우시관법적79.1%,차이균유통계학의의(P<0.001)。결론:미주응효잡식법대우보조진단HDN우우전통시관법,조작간단,모시단,결과역관찰,치득재림상상추엄。
Objective:To investigate value of micro column gel card method and test tube method in diagnosis ABO hemolytic disease of newborn.Method:278 blood samples of newborn suspected case of hemolytic disease of the newborn (HDN)with maternal fetal ABO blood incompatibility from July 2012 to July 2013 in department of pediatrics in our hospital were selected.The HDN three tests of direct Coombs test,serum free antibody,elution test were checked with the two methods of micro column gel card and test tube respectively.The detection results of two methods were compared.Result:In 278 cases,263 cases of clinically were confirmed.The positive rates of direct Coombs test in micro column gel card method was 76.4%,it was significantly higher than 59.3%in test tube method,the serum free antibody was 90.5%,it was significantly higher than 66.2%in test tube method,and elution test was 99.2%,it was significantly higher than 79.1%in test tube method,the differences were statistically significant(P<0.001).Conclusion:Micro column gel card method is simpler operation,rapid,easier to observe the results than the methods of test tube and is worth in clinical promotion.
