目的:评价噬菌体生物扩增技术(PhaB )在肺结核诊断中的应用。方法回顾性分析采用PhaB法、3D培养法、直接涂片法检测373份痰标本的抗酸杆菌结果。结果 PhaB法、3D培养法、直接涂片法检测抗酸杆菌的阳性率分别为53.6%、56.6%及20.1%。以3D培养法结果为标准,直接涂片法的敏感性、特异度、准确性分别为33.2%、95.7%及60.3%;PhaB法的敏感性、特异度、准确性分别为89.1%、93.6%及90.6%。直接涂片法与PhaB法检测结果比较,差异有统计学意义(χ2=94.7,P<0.01);PhaB法与3D培养法检测结果比较,差异无统计学意义(χ2=3.46,P>0.05)。结论 PhaB法检测结核分枝杆菌具有较高的敏感性、特异度、准确性,值得在临床推广。

목적:평개서균체생물확증기술(PhaB )재폐결핵진단중적응용。방법회고성분석채용PhaB법、3D배양법、직접도편법검측373빈담표본적항산간균결과。결과 PhaB법、3D배양법、직접도편법검측항산간균적양성솔분별위53.6%、56.6%급20.1%。이3D배양법결과위표준,직접도편법적민감성、특이도、준학성분별위33.2%、95.7%급60.3%;PhaB법적민감성、특이도、준학성분별위89.1%、93.6%급90.6%。직접도편법여PhaB법검측결과비교,차이유통계학의의(χ2=94.7,P<0.01);PhaB법여3D배양법검측결과비교,차이무통계학의의(χ2=3.46,P>0.05)。결론 PhaB법검측결핵분지간균구유교고적민감성、특이도、준학성,치득재림상추엄。
Objective To evaluate application of phage amplified biologically assay (PhaB) in the diagnosis of pulmonary tuber-culosis .Methods The results of acid-fast bacillus in 373 sputum specimens obtained by PhaB method ,3D culture and direct smear assay were retrospectively analyzed .Results The positive rates of acid-fast bacillus detected by PhaB method ,3D culture and the direct smear assay were 53 .6% ,56 .6% and 20 .1% ,respectively .Set the 3D culture result as the standard ,the sensitivity ,specifici-ty and accuracy of the direct smear assay were 33 .2% ,95 .7% and 60 .3% ,respectively ,and those of PhaB method were 89 .1% , 93 .6% and 90 .6% ,respectively .Compared the direct smear assay with PhaB method ,the difference showed statistical significance (χ2 =94 .7 ,P<0 .01) .Compared PhaB method with 3D culture ,the difference showed no statistical significance (χ2 =3 .46 ,P>0 .05) .Conclusion PhaB method has high sensitivity ,specificity and accuracy in Mycobacterium tuberculosis detection and is wor-thy of promotion .
