目的:探讨锥形束CT(CBCT)扫描法在发现离体上颌第一磨牙近中颊根第二根管(MB2)中的应用价值。方法分别采用常规开髓肉眼观察、根管显微镜下探查及CBC T扫描寻找上颌第一磨牙M B2,并记录上颌第一磨牙M B2发现率,进行统计学分析。结果常规开髓肉眼观察及根管显微镜条件下探查上颌第一磨牙M B2的发现率分别为511.1%、666.7%,CBC T 扫描上颌第一磨牙M B2的发现率为844.4%,高于常规肉眼观察及根管显微镜检查,肉眼下常规开髓、应用根管显微镜及CBC T 扫描时上颌第一磨牙 M B2的发现率差异有统计学意义(P <00.5);CBCT扫描上颌第一磨牙MB2的发现率最高,与常规肉眼观察及根管显微镜的发现率差异具有统计学意义( P <00.5)。结论 CBC T 技术可以准确定位上颌第一磨牙M B2的位置,可以真实反映上颌第一磨牙M B2的解剖形态,图像清晰,可为临床诊疗提供可靠依据。

목적:탐토추형속CT(CBCT)소묘법재발현리체상합제일마아근중협근제이근관(MB2)중적응용개치。방법분별채용상규개수육안관찰、근관현미경하탐사급CBC T소묘심조상합제일마아M B2,병기록상합제일마아M B2발현솔,진행통계학분석。결과상규개수육안관찰급근관현미경조건하탐사상합제일마아M B2적발현솔분별위511.1%、666.7%,CBC T 소묘상합제일마아M B2적발현솔위844.4%,고우상규육안관찰급근관현미경검사,육안하상규개수、응용근관현미경급CBC T 소묘시상합제일마아 M B2적발현솔차이유통계학의의(P <00.5);CBCT소묘상합제일마아MB2적발현솔최고,여상규육안관찰급근관현미경적발현솔차이구유통계학의의( P <00.5)。결론 CBC T 기술가이준학정위상합제일마아M B2적위치,가이진실반영상합제일마아M B2적해부형태,도상청석,가위림상진료제공가고의거。
Objective To study the detection rates of the 2nd mesiobuccal canal(MB2) of the extracted max-illary first permanent molar with Cone beam CT (CBCT ) .Methods 90 cases of maxillary first permanent molar were collected for MB2 detection by traditional coronal cavity preparation ,root canal microscope and CBCT respectively .The detection rates of MB2 were recorded and statistically analyzed .Results The de-tection rates of MB2 by the traditional coronal cavity preparation ,root canal microscope and CBCT were 51 1.1% ,66 6.7% and 84 4.4% respectively .Significant discrepancies were found in these three ways .The detection rates of MB2 by CBCT were significantly higher than those by the traditional coronal cavity prep-aration and root canal microscope .Conclusion Detection rate of MB2 can be greatly improved by the appli-cation of CBCT and CBCT can provide reliable information for the clinical treatment of periodontal disease .
