目的:利用NMR代谢组学技术分析哮喘肾虚痰瘀型与非肾虚痰瘀型患者血浆代谢化合物,开展中医哮喘“同病异证”性研究。方法分别取肾虚痰瘀型和非肾虚痰瘀型哮喘患者及健康人的血浆进行1 H-NM R检测,采用正交偏最小二乘判别(OPLS-DA)分析图谱。确定健康组与肾虚痰瘀型哮喘组、肾虚痰瘀型与非肾虚痰瘀型哮喘组血浆中的差异性代谢化合物。结果健康组与肾虚痰瘀型哮喘组、肾虚痰瘀型哮喘组与非肾虚痰瘀型哮喘组的代谢表型发生了显著变异,肾虚痰瘀型哮喘组与健康组比较,血浆中异亮氨酸、亮氨酸、缬氨酸、乳酸、丙氨酸、糖蛋白、谷氨酰胺、肉碱、酪氨酸、肌酸、组氨酸、鲨肌醇、甘氨酸、葡萄糖、苏氨酸、肌醇、1-甲基组氨酸降低,肌酸升高,差异有统计学意义(P <00.5);肾虚痰瘀型哮喘组与非肾虚痰瘀型哮喘组比较,肾虚痰瘀型哮喘组血浆中丙酮酸、甘氨酸升高;非肾虚痰瘀型哮喘组血浆中异亮氨酸、亮氨酸、缬氨酸、乳酸、丙氨酸、糖蛋白、丙酮、酪氨酸、1-甲基组氨酸升高,差异有统计学意义(P <00.5)。结论支气管哮喘肾虚痰瘀证的临床症状在微观生物学存在一定的物质基础,但仍需进一步研究与明确,中医支气管哮喘存在一定的再分性,中医支气管哮喘“同病异证”性及中医哮喘肾虚痰瘀证存在一定的生物学物质基础。

Objective To study different syndromes of one disease of asthma in traditional Chinese medicine by anylyzing the plasma metabolic compoundsmeans with nuclear magnetic resonance (NMR) .Methods Plasma samples from the asthma patients with syndromes of kidney deficiency and phlegm-stasis ,and asthma patients without the syndromes of kidney deficiency and phlegm-stasis and healthy people were de-tected and described with 1 H-NMR based on metabonomics ,and Orthogonal Partial Least-Squares Discrim-inant Analysis (OPLS-DA ) was adopted to analyze the data to determine the influential biomarkers by Pearson′ s-product-moment-correlation-coefficient and-2D-NM R . Results There were significant differences found in the metabolic phenotypes of the kidney deficiency and phlegm-stasis syndrome asthma patients and other syndrome plasma patients and healthy people ,and obvious classification was showed in the score-scattering-plot of OPLS-DA model .Compared with healthy people ,the asthma patients with the syndrome of kidney deficiency and phlegm-stasis had changes in the contents of plasma :alanine ,tyrosine and leucine ,citric acid ,threonine ,valine ,isoleucine ,histidine ,citrulline ,carnitine ,1-methyl histidine , pyruvic acid ,glycine ,decreased (P <0 0.5) .When kidney deficiency and phlegm-stasis syndrome asthma patients were compared with other syndrome plasma patients ,the content of 1-methyl histidine glucose , alpha ,beta glucose ,phenylalanine ,alanine ,acetone ,pyruvic acid ,glutamine ,creatine ,citric acid ,carni-tine ,shark inositol ,histidine in plasma were lower than other syndrome plasma ,and pyruvic acid ,glycine in plasma were higher than those of other syndrome plasma (P <0 0.5) .Conclusion Kidney-deficiency and phlegm-stasis syndrome has some certain material evidence in microscopic biology .Asthma in traditional Chinese medicine has different syndromes ,and the notion of “the same disease for different syndromes”has certain material evidence in microscopic biology .
