目的：探讨大子宫肌瘤剔除术前应用曲普瑞林对腹腔镜手术治疗效果的影响。方法：对有生育要求的大子宫肌瘤患者65例在月经第2天肌肉注射曲普瑞林3.75 mg，每28天1次，3个疗程后行腹腔镜下子宫肌瘤剔除术，与同期未使用曲普瑞林直接行腹腔镜下大子宫肌瘤剔除术的70例患者进行治疗效果对比。结果：两组均在腹腔镜下完成手术，无严重并发症发生；观察组手术时间、术中出血量、排气时间、盆腔引流量及术后住院时间均明显优于对照组，比较差异均有统计学意义（P<0.05）。结论：大子宫肌瘤剔除术前应用曲普瑞林利于腹腔镜手术治疗，临床效果明显，安全有效，值得临床推广。
목적：탐토대자궁기류척제술전응용곡보서림대복강경수술치료효과적영향。방법：대유생육요구적대자궁기류환자65례재월경제2천기육주사곡보서림3.75 mg，매28천1차，3개료정후행복강경하자궁기류척제술，여동기미사용곡보서림직접행복강경하대자궁기류척제술적70례환자진행치료효과대비。결과：량조균재복강경하완성수술，무엄중병발증발생；관찰조수술시간、술중출혈량、배기시간、분강인류량급술후주원시간균명현우우대조조，비교차이균유통계학의의（P<0.05）。결론：대자궁기류척제술전응용곡보서림리우복강경수술치료，림상효과명현，안전유효，치득림상추엄。
Objective:To investigate the operative effect of laparoscopic myomectomy in women with large leiomyoma.Method:Patients with large uterine fibroid who desire childbearing underwent laparoscopic myomectomy after three cycles preoperative triptorelin treatment,intramuscular injections 3.75 mg on the second day of menstrual cycle,once every 28 days.the other 70 cases who underwent laparoscopic myomectomy directly without triptorelin treatment were selected as control group,the clinical therapeutic effect of the two groups were analyzed.Result:Two groups underwent operation by laparoscopic myomectomy,the operation time,the amount of hemorrhage during the operation,the amount of pelvic draining,the exhaust time and the average time of hospitalization of the observation group were significantly better than those of the control group,the differences were statistically significant (P<0.05).Conclusion:The clinical effect of preoperative triptorelin treatment before laparoscopic myomectomy with large uterine fibroids is significant, safe and effective,which is worthy of promotion.
