目的:观察在延伸护理的理念指导下,采用的院内外联合的认知训练方式对轻度认知功能损害(MCI)患者认知功能的改善效果。方法选择M C I患者40例并均分为两组,实验组和对照组在出院前均进行认知训练。出院后,实验组继续进行认知功能训练,而对照组停止认知功能训练,并在入院时、3个月、6个月及12个月这4个时间点,两组的每一个患者进行认知功能评价,评价采用简易智力状态检查表(MMSE)、日常生活能力量表(ADL)。结果入院及3个月时,两组的MMSE及ADL评分差异均无显著意义(P>0.05);6个月时两组MMSE及ADL评分差异有显著意义(MMSE:P<0.05;ADL :P<0.01);12个月时两组MMSE及ADL评分差异有显著意义。实验组认知训练后与自身入院时比较,MMSE及ADL评分于第3个月时均较入院时差异有显著意义(P<0.05);第6个月至12个月时,差异更加明显(P<0.01)。结论在延伸护理的指导下,对MCI患者进行院内外联合的认知训练,可明显提高患者的认知功能,并较仅有院内认知训练的效果更佳。

Objective To study the improving effect of cognitive function for patients with mild cognitive impair-ment (MCI) by inside and outside the hospital training guided by the theory of extended care .Method 40 patients with MCI were randomly divided into experimental group and control group .20 patients in each group .The patients in both group was given cognitive training in hospital before discharge .The patients in experimental group was given continuously cognitive training after discharge .The patients in control group was not given cognitive training after discharge .The cognitive ability of the two groups was analyzed by mini-mental state examination (MMSE)and abili-ty of daily life(ADL) on hospital admission ,3rd month ,6th month ,12th month after admission .Result There was no significant difference of MMSE and ADL between the two groups on hospital admission and 3rd month .There was significant difference on 6th month ( MMSE:P<0 .05 ;ADL :P<0 .01) .There was no significance difference on 12th month (the P value reduced ) .There was significant difference from 3rd to 12th month of MMSE and ADL in experimental group self compared to the hospital admission .Conclusion The cognitive ability for patients with MCI could be improved by cognitive training with the guide of extended care ,which was better than the cognitive training only in hospital .
