目的:探讨儿童高原性心脏病的临床特点及治疗经验。方法回顾性分析32例高原性心脏病患儿的临床资料,所有患儿均为居住在青藏高原(海拔3000m以上)的儿童。结果32例患儿中,男18例、女14例,除1例外均<3岁;合并呼吸道感染30例,合并腹泻2例,有心功能不全15例。经持续低流量吸氧、降肺动脉高压、抗感染、对症支持治疗,合并心功能不全者加用强心、利尿治疗。30例好转,1例自动出院,1例因重症肺炎合并严重心功能衰竭死亡。结论高原性心脏病在高原儿童中也易发生,需要早期诊断,及时采取有效治疗,以降低病死率。

목적:탐토인동고원성심장병적림상특점급치료경험。방법회고성분석32례고원성심장병환인적림상자료,소유환인균위거주재청장고원(해발3000m이상)적인동。결과32례환인중,남18례、녀14례,제1예외균<3세;합병호흡도감염30례,합병복사2례,유심공능불전15례。경지속저류량흡양、강폐동맥고압、항감염、대증지지치료,합병심공능불전자가용강심、이뇨치료。30례호전,1례자동출원,1례인중증폐염합병엄중심공능쇠갈사망。결론고원성심장병재고원인동중야역발생,수요조기진단,급시채취유효치료,이강저병사솔。
Objective To investigate the clinical characteristic and therapeutic experience of high altitude heart disease in children. Methods The clinical data of 32 children with high altitude heart disease were retrospectively analyzed who lived in the Qinghai-Tibetan Plateau (3000 meters above the sea level). Results Among 32 children, 18 were males and 14 were fe-males. All children were less than 3 years old except one. Thirty children were complicated by respiratory tract infection, 2 by diarrhea and 15 by heart failure. Conventional treatments of continuous low flow oxygen, anti-pulmonary hypertension, anti-in-fection and supportive therapy were generally performed. If children were complicated by heart failure, cardiotonic and diuretic therapy were applied based on conventional treatments. After treatment, 30 children were improved;one child left hospital be-fore the completion of treatment;one died of severe pneumonia with severe heart failure. Conclusions The pediatric high alti-tude heart disease is one of the most common diseases in high altitude area. Early diagnosis, timely and effective treatment is helpful for reducing the case fatality rate.
