目的:探讨血糖波动对2型糖尿病患者肾病发生的影响。方法采用美敦力动态血糖监测系统(CGMS)对136例2型糖尿病患者进行连续72 h 动态血糖监测,通过尿白蛋白/肌酐测定值将所有糖尿病患者分为无糖尿病肾病组(NDN 组)和早期 DN 患者(DN 组),观察其72 h 平均血糖水平、血糖波动水平以及HbA1 c、血压和血脂谱等一般情况,并进行相关分析。结果(1)两组间血压、FBG 、24hMBG、MAGE、HbA1 c 等差异有统计学意义(P <0.05)。(2)回归分析显示24hMBG、MAGE 是早期糖尿病肾病发生的独立危险因素,OR 值分别为0.563、1.220(P 值分别为0.005和0.011)。结论血糖波动是糖尿病肾病发生发展的危险因素之一。

목적:탐토혈당파동대2형당뇨병환자신병발생적영향。방법채용미돈력동태혈당감측계통(CGMS)대136례2형당뇨병환자진행련속72 h 동태혈당감측,통과뇨백단백/기항측정치장소유당뇨병환자분위무당뇨병신병조(NDN 조)화조기 DN 환자(DN 조),관찰기72 h 평균혈당수평、혈당파동수평이급HbA1 c、혈압화혈지보등일반정황,병진행상관분석。결과(1)량조간혈압、FBG 、24hMBG、MAGE、HbA1 c 등차이유통계학의의(P <0.05)。(2)회귀분석현시24hMBG、MAGE 시조기당뇨병신병발생적독립위험인소,OR 치분별위0.563、1.220(P 치분별위0.005화0.011)。결론혈당파동시당뇨병신병발생발전적위험인소지일。
Objective To investigate the influence of the blood glucose fluctuation on the incidence of dia-betic nephropathy in patients with type 2 diabetes.Methods Continuous glucose monitoring system (CGMS)for 72 h was applied for 136 patients with type 2 diabetes mellitus.All diabetes mellitus were divided into non diabetic ne-phropathy group (NDN group)and patients with early DN (DN group)according to the ratio of urinary albumin to creatinine.The levels of 72 h average blood glucose,blood glucose fluctuation and HbA1 c,blood pressure,blood lipid were recorded and analysed.Results (1)There was significant difference of blood pressure,fasting blood glucose, the 24 h average blood glucose level(24hMBG),average blood glucose excursion amplitude(MAGE)and HbA1 c be-tween the two groups (P <0.05).(2)The results of Logistic analysis showed that 24hMBG and MAGE were inde-pendent risk factors to the incidence of early diabetic nephropathy,with the OR of 0.563 and 1.220(P =0.005and 0.011),respectively.Conclusion Glucose fluctuation is a important risk for the development of diabetic nephropathy.
