城镇职工基本医疗保险制度、新型农村合作医疗制度和城镇居民基本医疗保险制度三项制度构成了现阶段我国全民基本医疗保险体系的基本框架。可以预期，随着社会经济发展的城乡一体化将最终整合这种“破碎化”的医疗保障体系。本文以大城市典型样本---北京市的医疗保障制度的发展为研究对象，基于城镇化和人口老龄化的视角，初步探讨了“真正”城乡一体化的医疗保障制度发展现状和演进方向。

The basic medical insurance for urban employees, the new rural cooperative medical scheme and the basic medical insurance system for urban residents constitute the framework of current basic national medical insurance system in China. It can be expected that the “fragmentized” medical insurance system will finally be integrated by the integration of urban and rural development. Take the medical insurance system of Beijing ( a typical sample of big city) as the research object, making a preliminary study on the“real” medical insurance system’s current status and evolutional direction of the integration of urban and rural areas from the prospective of urbanization and the aging of population.
