目的:评估低氨基酸培养基和MH培养基对碳青霉烯类药物抗非发酵菌体外药敏检测结果的区别。方法:采用琼脂稀释法分别使用MH和低氨基酸培养基测定亚胺培南、美罗培南、帕尼培南等10种抗菌药物对我院临床分离60株铜绿假单胞菌和140株鲍曼不动杆菌的最低抑菌浓度(MIC),比较其结果的差异。结果:使用MH琼脂作为药敏培养基时,60株铜绿假单胞菌和140株鲍曼不动杆菌对亚胺培南、美罗培南和帕尼培南的敏感率分别为90%、85%和68.3%和15.7%、23.6%和15%;使用低氨基酸琼脂作为药敏培养基时,它们的敏感性分别上升到100%(P=0.272)、96.7%(P=0.053)、93.3%(P=0.001)和90.7%(P=0.001)、98.6%(P=0.001)和91.4%(P=0.001),其中铜绿假单胞菌对帕尼培南的敏感率上升有显著差异,而鲍曼不动杆菌对三者的敏感率上升均有显著差异。结论:我院分离的鲍曼不动杆菌对临床常用抗菌药物的耐药率较高。碳青霉烯类药物对铜绿假单胞菌和鲍曼不动杆菌的抗菌活性高于其它类抗菌药物。低氨基酸培养基,由于其碱性氨基酸浓度降低至与人血清中相近的浓度,适宜用于帕尼培南体外敏感性测定。

Objective: To evaluate the effects of low-amino-acid agar and Mueller-Hinton (MH) agar on the susceptibility of clinical non-fermentative isolates to three carbapenems. Methods:The activities of imipenem, meropenem, panipenem and so on were tested against 40 Pseudomonas aeruginosa and 160 Acinetobacter baumannii strains isolated from our hospital. The minimum inhibitory concentration (MIC) of each antimicrobial agent against these strains was determined by a standard agar dilution procedure on the MH and low-amino-acid agars, respectively. Results:The susceptibility of P. aeruginosa and A. baumannii to imipenem, meropenem, panipenem was 90%, 85%, 68.3%and 15.7%, 23.6%, 15%, respectively when they were incubated on MH agar, however, their susceptibility was enhanced to 100%(P=0.272), 96.7%(P=0.053), 93.3%(P=0.001) and 90.7% (P=0.001), 98.6% (P=0.001) and 91.4%(P=0.001) on low-amino-acid agar, respectively, which were statistically significant. Conclusion: Carbapenems had good antibacterial activity in vitro against P. aeruginosa and A. baumannii. Low concentrations of basic amino acid medium should be used in order to get consistent result between in vitro susceptibilities and clinical effect of panipenem against P. aeruginosa..
